缬沙坦对2型糖尿病肾病相关炎症因子的影响 被引量:4

Influence of valsartan to correlated inflammatory cytokines in diabetic nephropathy

摘要 目的:观察缬沙坦对2型糖尿病早期肾病相关炎症因子的影响及对糖尿病肾病的保护作用。方法:2型糖尿病早期肾病患者60例,随机分为常规治疗组30例,缬沙坦组30例,全自动生化分析仪测定血糖、血脂、血肌酐(Scr)、C反应蛋白(CRP)等,干式化学法测定24小时尿蛋白,放免法测定尿白蛋白排泄率(UAER),ELISA法测定血清转化生长因子β1(TGF-β1)、白介素-6(IL-6)、肿瘤坏死因子α(TNF-α)、白介素-18(IL-18)。结果:(1)缬沙坦组治疗后ALB/Cr、24小时尿蛋白、UAER明显降低,而治疗前后血压无明显差异。(2)缬沙坦组治疗后CRP、TGF-β1、IL-6、IL-18、TNF-α等均显著降低。结论:缬沙坦在降压作用之外还能减轻炎症反应,减少蛋白尿,延缓糖尿病肾病的发生、发展。 Objective: To study the influence of valsartan to correlated inflammatory cytokine in early stage of type 2 diabetic nephropathy and the protection to diabetic nephropathy.Methods: Sixty patients with early stage of type 2 diabetic nephropathy were divided randomly into two groups,including 30 patients with conventional therapy,30 patients with therapy of valsartan.Blood glucose,blood fat,serum creatinine and C reactive protein(CRP) were measured by automatic biochemistry analyzer.Twenty-four hours urine protein was measured by dry-type chemical method.Urinary albumin excretion rate(UAER) was measured by radio-immunity method.Serum transforming growth factor β1(TGF-β1),interlukin-6(IL-6),tumor necrosis factor-α(TNF-α)and interlukin-18(IL-18) were measured by enzyme linked immunosorbent assay(ELISA) method.Results:(1) ALB/Cr,24-hour urine protein and UAER of the group with therapy of valsartan were significantly decreased after therapy and no significant difference of blood pressure could be seen.(2) Level of serum CRP,TGF-β1,IL-6,IL-18 and TNF-α of group with therapy of valsartan were significantly decreased after the therapy.Conclusion: besides of lowering blood pressure,valsartan can also postpone the development of diabetic nephropathy by decreasing the level of correlated serum inflammatory cytokines and urine protein.
