摘要
目的:探讨血清基质金属蛋白酶(MMP)-9与B型脑利钠肽(BNP)在急性心肌梗塞(AMI)患者急诊介入治疗前、后含量的变化及其临床意义。方法:选择行急诊经皮冠状动脉介入(PCI)术的AMI患者60例为PCI组,冠状动脉造影正常者30例为正常对照组。采用酶联免疫吸附法(ELISA)检测AMI患者术前及术后第1d血浆MMP-9及BNP的含量,并与对照组检测值进行比较。结果:AMI患者PCI术前MMP-9[(711±169)μg/L∶(216±49)μg/L]及BNP[(329±85)ng/L)∶(29±10)ng/L]水平均较正常对照组明显升高(P均<0.01),且BNP与MMP-9水平呈正相关(r=0.38,P<0.05)。与术前比较,PCI术后血浆BNP明显下降[(329±85)ng/L∶(278±82)ng/L],MMP-9水平明显升高[(711±169)μg/L∶(763±189)μg/L]。结论:MMP-9水平增加可能是心肌缺血再灌注损伤的机制之一,可能与缺血再灌注后的炎症损伤有关。而术前B型脑利钠肽水平升高与炎症损伤有关。
Objective: To explore the changes and clinical significance of matrix metalloproteinases- 9 (MMP -9) and B --type brain natriuretic peptide (BNP) levels in patients with acule myocardial infarction (AMI) before and after PCI. Methods: A total of 60 patients with AM1 who received percutaneous coronary intervention (PC1) were chosen as PCI group, and 30 cases with normal coronary angiography were regard as normal control group. The serum MMP 9 and BNP levels of AMI patients before and 1d after PCI were measured by enzyme-linked immunosorbent assay (ELISA). The results will be compared with those of normal control group. Results: The serum levels of MMP-9 [ (711 ±169)μg/L vs. (216±49)μg/L] and BNP [ (329±85) ng/L) vs. (29±10) ng/L] in AMI patiems before PC1 were significantly higher than those of normal control group (P〈O. 01 all). And the serum BNP level was positive correlated with MMP--9 (r= 0.38, P〈0.05). Compared with preoperative, the plasma BNP level decreased significantly [ (329 ±85) ng/L vs. (278±82) ng/L ], the MMP 9 level increased significantly [ (711±169) lxg/L vs. (763±E189) μg/ L] after PCI. Conclusion: The increase of MMP--9 level may be one of the molecular mechanisms of myocardial ische- mia reperfusion injury, it may be related with inflammatory injuries after myocardial ischemia reperfusion. The increase of serum BNP level before PCI is related with inflammatory injuries.
