摘要
目的 研究膝关节骨显像半定量分析的方法,建立其半定量分析的参考值,并初步探讨其在膝关节疾病诊断中的应用价值.方法 对临床上无膝关节及股骨疾患病史,且X线检查未见异常的患者分别按照性别分为男性组、女性组,按年龄段分成7组.所有患者均行全身骨显像及膝关节局部放射性骨显像,根据膝关节解剖结构将其分为4个感兴趣区,即胫骨内侧髁(a)、外侧髁(b)和股骨内上髁(c)、股骨外上髁(d)4个部分,于以上4个部位及同侧股骨中段(e)选取面积相等的圆形感兴趣区,测量其放射性计数,并计算膝关节感兴趣区放射性计数与同侧股骨中段感兴趣区放射性计数比值.结果 ①左右两侧同一感兴趣区的放射性比值及同一膝关节4个感兴趣区放射性比值在统计学上无明显差异(P〉0.05);②年龄小于60岁时,男女之间同一感兴趣区的放射性比值之间无差别(P〉0.05),于〉60岁后差异具有显著性意义(P〈0.05).③男性组:18~20岁年龄段除与21~30年龄段无差异性外(P〉0.05),与其他各年龄段差别亦均有明显统计学意义(P〈0.01).结论 ①膝关节放射性核素骨显像定量分析法显示膝关节感兴趣区放射性计数与股骨中段放射性计数比值的大小受年龄和性别等因素的影响,与左右位置和膝关节感兴趣区部位无明显关系.②根据统计的参考值范围可以作为膝关节OA及其他一些膝关节疾病诊断的参考依据.
Objective To establish normal reference value of radioactivity ratio of regions of interest of knee joint/the middle part of femur, and to explore how much age and gender will affect the ratios. Methods According to gender and the stage of age,it divided all the patients who had radionuclide imaging of bone in our department and all had no history of pain in the knee joint and femur,and were also exempt from diseases by X-ray into the following groups : All patients had both whole body imaging and local imaging of knee joint 3 hours after intravenous injection of 99mTc-MDP. According to the anatomic structure of knee joint, we divided it into four regions of interest ( ROI ) which are a, b, c and d, and figure out the radioactivity ratio of regions of interest in knee joint / the middle part of femur ( e ). At the same time,54 patients suffering frum OA of knee were included in this study. Results No difference has been found between the right and the left knee joint of the symmetrical ROI and among the four ROI in the same knee joint. No difference has been found among the groups of 51 to 60,60 to 70 and older than 70 ( P 〉 0.05 ). The results indicated that all 54 OA patients who had local radionuclide imaging of knee joint and were given quantitative analysis. There were 159 ROIs with radioactivity ratios over the normal reference value of the related gender and age stage. Conclusion The results of quantitative method in radionuclide imaging of knee joint indicates that the site of the ROI may have little effect on the radioactivity ratios ,but the gender and age contribute significantly to the radioactivity ratios. The reference value of 95% confidence interval in each ROI of every age groups can be applied to diagnose the OA of the knee joint and other diseases in knee joint too.
