高血压患者糖化血红蛋白与微量白蛋白尿的关系及意义 被引量:7

Relationship between microalbuminuria and hemoglobin A1c in patients with hypertension

摘要 目的 探讨原发性高血压患者糖化血红蛋白(HbAlc)与微虽白蛋白尿(MAU)的关系,以及MAU的危险因素.方法 选取原发性高血压患者89例,按血压水平分为3组,高血压1级组10例,高血压2级组34例,高血压3级组45例,检测TG、TC、低密度脂蛋白胆同醇(LDL-C)、高密度脂蛋白肌固醇(HDL-C)、高敏C反应蛋白(hs-CRP)、肌酐(Scr)、尿素氮(BUN)、HbAlc、尿微最白蛋白(UAlb)、腰臀比(WHR)和体质指数(BMI),询问有无吸烟、饮酒、糖尿病、冠心病病史等危险因素.结果 3组比较,年龄、性别、吸烟、饮酒、糖尿病、冠心病病史、高血压病程、TG、TC、LDL-C、HDL-C、Scr、BUN、WHR和BMI差异均无统计学意义 而3组间UAlb、hs-CRP和HbAlc差异均有统计学意义,且高血压3级组较1级组和2级组明显升高 hs-CRP与UAlb、HbAlc呈正相关(r=0.323,P=0.004 r=0.283,P=0.013),UAlb与HbAlc亦呈正相关(r=0.599,P=0.000) Logistic多元回归分析显示,Scr(OR=1.052,95%CI:0.997~1.110,P=0.043)、HbAlc(OR=2.475,95%CI:1.588~3.856,P=0.000)进入回归方程,是MAU的独立危险因素.结论 原发性高血压患者UAlb、hs-CRP和HbAlc互为相关,Scr、HbAlc是高血压患者MAU的独立危险因素.UAlb和HbAlc用于评估高血压患者靶器官损害的程度及治疗效果具有重要意义. Objective To investigate the relationship between microalbuminuria and hemoglobin Ale in patients with essential hypertension and to evaluate the risk factors of microalbuminuria. Methods Lipids, hs-CRP, UAlb, Scr, BUN, HbAlc, WHR, BMI, history of smoking, drinking, diabetes mellitus and coronary heart disease were analyzed in 89 patients with essential hypertension, which were classified based on blood pressure level : 10 in group 1, 34 in group 2, 45 in group 3. Results There was no significant differences in age, TC, TG, LDL-C, HDL-C, Scr, BUN, WHR, BMI, duration of hypertension and ratio of diabetes, coronary heart disease, gender, drinking, smoking among patients in three groups. Compared with hypertensive groupl and group 2, UAlb, hs-CRP, HbAlc were significant higher in patients in group 3 ; Hs-CRP was positively correlated with UAlb and HbA1 e ( r = 0. 323, P = 0. 004 ; r = 0. 283, P = 0.013 ) , UAlb was also positively correlated with HbAI c ( r = O. 599, P = 0. 000 ) ; Logistic regression analysis showed that Scr( OR = 1. 052, 95 % CI:0. 997-1.110, P = 0. 043 ) , HbA1 c ( OR = 2. 475,95 % CI: 1. 588-3. 856, P = 0. 000 ) were significantly correlated to microalbuminuria. Conclusions MAU, hs-CRP, HbAlc are positively correlated each other; Scr, HbAlc are risk factors of microalbuminuria; detection of microalbuminuria and HbAlc is an important access to target organ damage in patients with essential hypertension.
