站立位和蹲踞位上肢血氧饱和度差值在评估法洛氏四联症心脏功能中的作用

摘要 目的探讨先天性心脏病法洛氏四联症患儿在站立位和蹲踞位不同体位上肢血氧饱和度值的差值与左心室舒张末容积指数和肺动脉指数之间的关系。方法 19例先天性心脏病法洛氏四联症患儿(男性11名,女性8名,平均年龄6岁)接受实验。每位患儿于左手食指检测经皮血氧饱和度,分别在站立位和蹲踞位取值。分析站立位和蹲踞位血氧饱和度差值与左心室舒张末容积指数和肺动脉指数的相关性。结果站立位的血氧饱和度(79±4)%,蹲踞位(84±4)%,差值为(5±3)%,左心舒张末容积指数(41±5)ml/m2,肺动脉指数(188±37)mm2/m2,血氧饱和度差值与左心室舒张末容积指数正相关(r=0.854,P<0.05),蹲踞位SPO2与肺动脉指数正相关(r=0.641,P<0.05)。结论对于法洛氏四联症患儿,站立位和蹲踞位上肢血氧饱和度差值与左心舒张末容积指数有正相关关系,对于评估患儿左心功能是一个有意义的补充依据。 Objective To explore the relationship of the difference of transcutaneous pulse oxygen saturation in the standing position and squatting position(ΔSPO2)with left ventricular end-diastolic volume index(LVEDVI)and Nakata index in tetralogy of Fallot(TOF)ptients.Methods Nineteen TOF patients(11 boys and 8 girls,median age of 6 years)were selected for this study.The transcutaneous pulse oxygen saturation of every patient was assessed with a pulse oxymeter placed on the second left finger,first in a standing position and then in a squatting position.Correlation analysis of ΔSPO2 and the LVEDVI or Nakata index was conducted.Results The mean SPO2 was(79±4)% in the standing position and(84±4)% in squatting position,and the mean ΔSPO2 was(5±3)%.The LVEDVI was(41±5)ml/m^2 and the Nakata index was(188±37)mm^2/m^2.ΔSPO2 was correlated with both LVEDVI(r=0.854,P〈0.05),and the SPO2 in the squatting position was positively correlated with the Nakata index(r=0.641,P〈0.05).Conclusion For patients with TOF,the ΔSPO2 between standing and squatting positions difference ΔSPO2 might be a supplemental marker for evaluating the left ventricular function.
