颧骨复合体骨性架构的数据测量及临床评价

Clinical evaluation and data measurement of osseous structure of zygomatic complex

摘要 目的 对正常头颅标本和临床上颧骨高凸患者的颧骨复合体进行测量和分析,以提供颧骨复合体相关手术的评价方法.方法 自2005年1月至2007年6月,将解剖学专家选取的颧骨复合体骨性架构比例正常的45例成人女性尸体的头颅,及整形外科医师选取的颧骨复合体高凸畸形的24例成人女性患者,分为头颅标本A组和临床患者B组,分别测量两组研究对象面中部轮廓的8个相关项目.A组行直接测量,B组行三维CT测量,将所测得的数据行Bayes判别分析,利用判别函数方程分析结果.结果 Bayes判别函数:A组=-114 268-4.945×X1+16.134×X2+1.077 × X3-0.453×X4+307 910.7 × X5+6.858×X6+1727.174×X7-2343.358×X8;B组=-114 512-5.017×X1+16.325×X2+1.114×X3-0.294×X4+308 205.1×X5+6.939×X6+1728.865×X7-2345.710×X8;判别符合率为81.2%.结论 本研究对颧骨复合体的测量和方法的分析,以及临床手术有一定的指导意义,但需进一步的基础性和统计学的研究加以完善. Objective To measure and analyze zygomatic complex of normal head specimen and abnormal protrusion zygoma to supply the evaluation method of operation concerning zygomatic complex. Methods From January 2005 to June 2007, 45 skulls with normal proportion zygomatic complex selected from adult female corpse by anatomical experts and 24 female patients with abnormal protrusion zygomatic complex selected and judged by the experienced plastic surgeons were divided into Group A (head specimen) and Group B (patients). Eight indexes concerning facial contour were measured on the skulls in Group A and the three dimensional CT in Group B. Finally the measured data were analyzed with Bayes discriminant analysis and functional equation method. Results Bayes diseriminant analysis : Group A = - 114 268 - 4.945 × X1 + 16. 134 × X2 + 1.077 ×X3 -0. 453 × X4 + 307 910.7 × X5 + 6. 858 × X6 + 1727. 174 × X7 - 2343. 358 × XS; Group B= -114512-5.017×X1 +16.325×X2+1.114×X3-0.294×X4+308205.1 ×X5 +6.939 ×X6+ 1728. 865 ×X7 - 2345. 710 × X8. The coincidence rate was 81.2%. Conclusion The method of measurement and analysis for the zygomatie complex could be used for the instruction for clinical surgery, but additional basic researches and long term statistical studies are needed in the future.
