摘要
脑转移瘤是成人最常见的颅内肿瘤,近年来发病率逐年上升。由于传统药物和观念的限制,化疗在脑转移癌治疗中的作用一直未被重视。近年来随着一些新药的尝试和观念的改变,转移瘤化疗的研究有了很大的进步。本文综述了近年来国内外化疗及化疗加全脑放疗治疗脑转移瘤相关的临床研究,显示化疗对敏感肿瘤引起的脑转移有一定疗效;对一些化疗不敏感肿瘤,全脑放疗加化疗没有明确而有力的生存期的获益,但增加了患者的反应率。目前缺少有关脑转移瘤化疗的随机大样本对照研究,这也是今后脑转移瘤化疗的研究方向。分子靶向药物治疗也是今后治疗脑转移瘤的一个热点。
Brain metastases are the most common intracranial tumors of adults, and the incidence of brain metastases is increasing in recent years. Because of the limitation of traditional cytotoxic agents and the concept, the role of chemotherapy in the treatment of brain metastases remains controversial. With the development of the new agents, increasing number of studies about chemotherapy for brain metastases were carried out. In this articles, the literatures about chemotherapy and chemotherapy plus whole brain radiotherapy （WBRT） for brain metastas were reviewed. Some studies have shown that chemotherapy was effective for some brain metastases. The addition of chemotherapy to WBRT for the chemoresistant brain metastases improved response rate but lack of clear and robust survival benefit. It is necessary to conduct randomized controlled trials to investigate the effectiveness of chemotherapy for brain metastases chemotherapy. In addition, molecular target therapy maybe another hot spot for the treatment of brain metastases.
