高频超声评价指导手部肌腱损伤患者康复治疗的临床价值 被引量:5

摘要 目的探讨高频超声在评价手部肌腱损伤术后综合康复治疗效果的价值。方法对82例(156指)手部肌腱损伤患者,依患者客观条件和个人意愿分为康复治疗组42例(93指),手术缝合后由康复技师进行规范的康复训练;对照组40例(63指),手术缝合后常规护理,采用传统的早期制动,3周后自己进行功能锻炼。所有患者均于术前、术后1个月、3个月、6个月进行超声复查。结果 82例(156指)患者术前超声检查结果均与术中探察结果一致;肌腱损伤后断端及吻合口的形态及内部回声随时间的推移而改变;康复治疗组与对照组相比,患者肌腱粘连程度及范围较轻,康复过程较短,手部功能恢复较快。结论高频超声对手部肌腱损伤术前诊断、术后康复效果评价有重要价值;对临床康复治疗方案选择及变更有重要指导意义。 Objective To explore comprehensive evaluation value of high frequency ultrasonography in hand tendon injuries postoperatively. Methods There were 82 cases (156 fingers) whose tendon in hands were injured.According to objective conditions and individual aspiration, patients were divided into treatment group (42 cases,93 fingers) and control group(40 cases,63 fingers). Treatment group were received rehabilitation training by standard technician after surgical suture. Control group were received conventional care after surgical suture and used the traditional brake, then processed functional exercise by themselves after three weeks. All of the patients reviewed by ultrasonography at regular intervals. Results Preoperative ultrasonography results of 82 cases (156 fingers) were detected with the same results of intraoperative detection.Cut ends and anastomotic form and echogenicity after the tendon injuries were recovered with the passage of time.Compared with the control group, scope and extent tendon adhesion of the treatment group were lighter, rehabilitation process was shorter and hand function recovered quickly. Conclusion High frequency ultrasonography had important value in the preoperative diagnosis of finger tendon injury and postoperative rehabilitation.It also had important significance for clinical rehabilitation scheme selection.
