新生儿重症监护病房鲍曼不动杆菌耐药性监测及AmpC酶检测

Detection and antimicrobial susceptibility testing for AmpC β-lactamase producing strains of acinetobacter baumannii isolated from neonatal intensive care units

摘要 目的了解深圳妇幼保健院新生儿重症监护病房(NICU)新生儿感染鲍曼不动杆菌耐药情况及其产AmpCβ酶的情况。方法收集2006年1月-2009年12月NICU新生儿临床样本分离鲍曼不动杆菌109株,采用纸片扩散法检测其对常用抗生素的耐药性,采用改良三维试验检测其是否产AmpC酶。结果 2006年1月-2009年12月NICU鲍曼不动杆菌分离率呈逐年增长趋势,产AmpC酶菌株检出率为35.8%(39/109),产AmpC酶菌株对第三代头孢菌素耐药率为92.3%-100.0%,对亚胺培南、阿米卡星、环丙沙星、哌拉西林/他唑巴坦和头孢哌酮/舒巴坦耐药率较低为7.6%-33.0%,产AmpC酶菌株耐药率明显高于非产AmpC酶菌株。结论 NICU鲍曼不动杆菌产AmpC酶率较高,其对常用抗生素耐药率高,且呈多重耐药,应引起临床高度重视,并加强AmpC酶的检测。 Objective To investigate the proportion and drug resistance of AmpC β-1actarnases producing strains for acinetobacter baumannii isolated from neonatal intensive care units(NICU).Methods Acinetobacter baumannii strains were isolated from NICU during January,2006 to December,2009.Drug susceptibility test was evaluated by Kirby-Bauer procedure.Isolates with the production of AmpC β-lactamases were detected by the improved cefotaxime three-dimensional test.Results 109 isolates of acinetobacter baumannii were confirmed during the period,and among which 35.8%(39/109) were AmpC β-lactamases producing strains.The drug resistance rates of AmpC β-lactamases producing strains were 92.3%-100% to the third-generation of cephalosporins,while the rates were 7.6%-33.0% to imipenem,amikacin,ciprofloxacin,piperacillin/tazobactant and cefperazone/sulbactam.The drug resistance rates of acinetobacter baumannii were higher in AmpC β-lactamases producing isolates than that in no AmpC β-lactamases producing ones.Conclusion NICU isolated acinetobacter baumannii had high proportion of AmpC β-lactamases producing strains,which were resistant to some commonly used antibiotics,especially the third and fourth generation of cephalothin,or had multiple drug resistance.Additional attention should be given to antibiotics selection for the clinical treatment.
