摘要
患者男,60岁。全身红斑丘疹伴剧痒6月,视力下降2周。头面及躯干部见大片浸润性红斑和斑块,周边界清,似体股癣状,球结膜充血;RPR1:64(+),TPPA(+)。皮损组织病理示:真皮浅层小血管浆细胞、淋巴细胞及嗜酸性粒细胞浸润。苄星青霉素治疗有效,随访3个月,血清RPR1:16(+),视力恢复。诊断:二期梅毒。
A 60-year-old male patient was referred to us because of erythema,papular with itching all over the body for 6 months and accompanied by impaired vision for 2 weeks.Physical examination showed infiltrating erythema,plaque,with distinct margin which seemed to tinea corporis or tinea cruris,over the skin of head,face and trunk accompanied by bulbar conjunctiva hyperemia.Rapid plasma regain （RPR） test 1：64 （＋）;treponema pallidum particle agglutination （TPPA） test （＋）.Histopathology examination revealed infiltration of plasmocyte,lymphocyte,eosinophil in the upper dermis.Treatment with benzathine was effective.At the end of the next 3 months′ follow-up,the titer of RPR reduced to 1：16 （＋） accompanied by vesion recuperation.A diagnosis of secondary syphilis was made.
