摘要
目的探讨射频热疗联合同步放化疗治疗局部晚期非小细胞肺癌的临床疗效及对机体免疫功能的影响。方法不能手术的局部晚期非小细胞肺癌65例,随机分为治疗组(33例),对照组(32例),治疗组给予射频热疗联合同步放化疗,对照组给予同步放化疗。评价疗效、毒副反应和免疫功能变化。结果治疗组:CR5例,PR23例,SD4例,PD1例,有效率为84.9%;对照组:CR4例,PR20例,SD6例,PD2例,有效率为75.0%;两组有效率比较差异无统计学意义(P>0.05)。治疗组的毒副反应明显低于对照组(P<0.05)。治疗组CD3+T细胞、CD4+T细胞、CD4+/CD8+治疗前后无明显变化(P>0.05),NK细胞明显升高(P<0.05);对照组治疗后CD4+T细胞、CD4+T细胞、CD4+/CD8+明显降低(P<0.05),NK细胞较治疗前明显升高(P<0.05)。治疗组经治疗后Th1型细胞因子明显升高(P<0.05),Th2型细胞因子明显降低(P<0.05);而对照组结果正好相反。结论热疗联合同步放化疗治疗局部晚期非小细胞肺癌疗效较好,患者可耐受,热疗能减轻同步放化疗的毒副反应及其对机体免疫功能的影响。
Objective To study the recent effect and variance in immunocyte and cytokine of hyperthermia plus concurrent chemoradiotherapy for locally advanced non-small cell lung cancer.Methods The 65 patients with locally advanced non-small cell lung cancer were enrolled in this study,the 32 patients in the control group received chemotherapy plus radiotherapy,the 33 patients in the treatment group received the same treatment plus hyperthermia.The effects,toxicities and immunologic function changes were respectively evaluated in the two groups.Results In the treatment group,5 patients achieved CR,23 patients PR,4 patients SD,1 patient PD,the overall response rate was 84.9% ;while 4 patients CR,20 patients PR,6 patients SD,2 patients PD,the overall response rate was 75.0% in control group;no significant difference was found between the two groups(P 0.05).The toxicities in the treatment group were lower than those in the control group(P 0.05).There were no significant changes in the proportion of CD 3+ and CD 4+ T-lymphocytes and the ratio of CD 4+ /CD 8+(P 0.05),but the number of NK cells significantly increased(P 0.05) after the treatment in the treatment group.The proportion of CD 3+ and CD 4+ T-lymphocytes and the ratio of CD 4+ /CD 8+ significantly declined after treatment,and the number of NK cells had significant changes in the control group(P 0.05).In the treatment group,the concentration of Th1 type cytokines were significantly higher,and that of Th2 type cytokines were significantly lower after hyperthermia than before hyperthermia(P 0.05).In the control group,the result was contrary to that of the treatment group.Conclusion Hyperthermia plus concurrent chemoradiotherapy for locally advanced non-small cell lung cancer is effective and well tolerated.Hyperthermia significantly lessens the toxicities of concurrent chemoradiotherapy and enhance patients immunologic function.
