摘要
目的比较Kaneda器械对不同程度胸腰段脊柱损伤提供的生物力学稳定能力及前纵韧带在Kaneda器械中的作用。方法采用5例胸腰段脊柱制备脊柱损伤标本,用传感器及电测技术,计算轴压、前屈、后伸、侧弯、旋转5种运动方式的平均顺应系数(f)。结果前中柱损伤Kaneda器械固定后,5种运动方式的f皆高于完整脊柱水平(P<0.05)。前纵韧带断裂后,损伤脊柱的轴压、前屈、后伸的f较前下降(P<0.05)。三柱损伤时,前屈运动明显失稳,后伸、侧弯、旋转的f也较前下降(P<0.05)。结论Kaneda器械能够给前中柱损伤脊柱提供优于完整脊柱的力学稳定性;保留前纵韧带有利于提高Kaneda器械的力学效益;Kaneda器械难以维持三柱损伤脊柱的稳定性。
Purpose To study the threedimensional stability of the spine injuries of different
degree treated by the Kaneda anterior device. Methods Five intact cadaveric thoracolumbar
specimens were used.They were measured by 3degree of freedom electrogniometer force and
displacement transducers for the relative rotation and horizontal translation between T 11
and L 1 vertebra in the bending direction.The mean flexibility (f) was computed. Results In the
damaged anterior and middle column of the spine, Kaneda could provide good stability in
compressive loading,flexion,extension,lateral bending and rotation( P <0.05).Cutting the
anterior longitudinal ligament could significantly decline the mechanical stability of
compressive loading,flexion,and extension of the damaged spine ( P <0.05).In the damaged
three columns of the spine,flexion was more unstable and the f in extension,lateral bending and
rotation were declined( P <0.05). Conclusions The Kaneda anterior device can increase the
mechanical stability of the spine with the damaged anterior and middle column. The intact
anterior longitudinal ligament is important for mechanical efficiency of Kaneda.Kaneda shows
poor efficiency in the damaged three columns of spine.
