摘要
目的:探讨神经电生理检测在糖尿病合并周围性面神经炎早期诊治中的价值。方法:将合并糖尿病的周围性面神经炎患者32例作为糖尿病组,与非合并糖尿病的周围性面神经炎患者30例作为对照组。在发病4-7d内进行瞬目反射(BR)、肌电图(EMG)和面神经电图(ENG)检查,比较分析其临床资料。结果:所有患者全部BR异常,两组间比较糖尿病组潜伏期比对照组延长,具有显著差异(P<0.05)。糖尿病组EMG检测异常率为62.5%,对照组EMG检测异常率为56.7%,两组之间EMG检测结果对比具有显著差异(P<0.05)。糖尿病组ENG检测异常率为34.4%,患侧M波潜伏期明显长于健侧(3.46±0.79 ms,P<0.05);对照组ENG检测异常率为36.7%,患侧M波潜伏期明显长于健侧(3.12±0.31 ms,P<0.05);两组之间ENG检测结果对比无显著差异(P>0.05)。结论:BR是面神经炎早期诊断最为敏感的神经电生理检测指标;BR在糖尿病性面神经炎早期诊断中更能反映面神经损害的程度;EMG在糖尿病性面神经炎早期诊断中较为敏感;ENG在糖尿病性面神经炎早期诊断中敏感性较差。
Objective: To explore the diagnostic significance of electroneurophysiology in patients with early stage of diabetic peripheral facial paralysis.Method:The blink reflex(BR),electromyography(EMG) and electroneurography(ENG) had been detected in 32 patients with peripheral facial paralysis complicated diabetes mellitus(DM) in the early stage from 4 days to 7 days.The contrast group was composed of 30 patients with peripheral facial paralysis without diabetes mellitus.Result:①Blink reflex(BR): In all the patients,the abnormal rate was 100%.In DM,the ipsilateral R1,R2 and R2’ waves were absent in 12 cases,and the latencies of R1,R2 and R2’ were prolonged in 20 cases.In the contrast group,the ipsilateral R1,R2 and R2’ waves were absent in 9 cases,and the latencies of R1,R2 and R2’ were prolonged in 21 cases.There was a marked difference about BR between the two groups(P〈0.05).②Electromyography(EMG): In DM,the abnormal rate was 62.5%.Detection of EMG displayed that polyphase wave increased,recruitment potential reduced and spontaneous potential appeared on the ill side.In the contrast group,the abnormal rate was 56.7%.There was a marked difference about EMG between the two groups(P〈0.05).③Electroneurography(ENG): In DM,the abnormal rate was 34.4%,but the latent period M wave on the ill side was prolonged.There was a marked difference about the latent period M wave between the ill side and the healthy side(P〈0.05).In the contrast group,the abnormal rate was 36.7%,but the latent period M wave on the ill side was prolonged.There was a marked difference about the latent period M wave between the ill side and the healthy side(P〈0.05).There was not a marked difference about ENG between the two groups(P〉0.05).Conclusion:①Blink reflex could be used as an objective index for the early diagnosis of facial paralysis.②In the early stage of diabetic peripheral facial paralysis,Blink reflex could reflect the extent of the damage about facial nerve.③EMG could be used as the secondary index for the early diagnosis of diabetic peripheral facial paralysis.④ENG could not be used as a sensitive index during the early diagnosis of diabetic peripheral facial paralysis.
