过敏性哮喘患者外周血Th17细胞及IL-17水平的变化 被引量:23

摘要 目的:探讨Th17细胞及其细胞因子IL-17水平在过敏性哮喘患者外周血中的变化及意义。方法:通过测定屋尘螨特异性IgE(sIgE)而选取19例轻度、16例中重度过敏性哮喘患者作为研究对象,并行肺功能及哮喘控制问卷(ACQ)调查,18名正常人作为对照。采用流式细胞术检测外周血Th17细胞的百分率,酶联免疫吸附试验(ELISA)检测血浆以及重组屋尘螨抗原蛋白1(rDerp1)刺激48h后外周血单个核细胞(PBMC)上清液中的IL-17水平。结果:中重度过敏性哮喘患者外周血Th17细胞百分率、血浆以及rDerp1刺激后PBMC上清液中的IL-17水平高于轻度哮喘患者(P值分别为<0.01、<0.01、<0.05)及正常对照组(P值分别为<0.01、<0.01、<0.05)。哮喘患者外周血中Th17细胞百分率、血浆IL-17水平与第1秒用力呼气容积(FEV1)占预计值百分率(%)呈负相关,与ACQ平均分呈正相关(P值均<0.01)。结论:Th17细胞应答增强可能促进哮喘加重,并影响其症状控制。 Objective To investigate the percentage of Th17 cells and expression of IL-17 in peripheral blood of patients with allergic asthma and its clinical significance. Methods Nineteen patients with mild allergic asthma and 16 patients with moderate-severe allergic asthma selected by house dust mites specific IgE and 18 healthy donors were enrolled. Pulmonary function and Asthma Control Questionnaire (ACQ) were examined. Percentage of peripheral blood Th17 cell was determined by flow cytometry. IL-17 levels in plasma and peripheral blood mononuclear cell (PBMC) supernatant 48 hours after stimulation by recombinant house dust mites antigenic protein (rDer p1) were measured by ELISA. Results The percentage of Th 17 cell in peripheral blood of patients with moderate-severe asthma was higher than those in patients with mild asthma (P0.01) and healthy donors (P0.01). The levels of IL-17 in plasma and peripheral blood mononuclear cell supernatant in patients with moderate-severe asthma were higher than those in patients with mild asthma (P0.01, P0.05) and healthy donors (P0.01, P0.05). Percentage of Th17 cell and plasma IL-17 level were negatively correlated with FEV1 (% of predicted value) and positively correlated with mean scores of ACQ (all P 0.01). Conclusions Enhanced response of Th17 cell can aggravate asthma and affect the control of asthmatic symptom.
