食管癌组织DcR3、MMP-2表达及其对生存率的影响 被引量:6

Study on the expression level of DcR3 and MMP-2 in esophageal carcinoma and its impact on survival of the patients

摘要 背景与目的:诱骗受体3(decoy receptor 3,DcR3)蛋白是一种肿瘤坏死因子,基质金属蛋白酶-2(matrix metalloproteinase-2,MMP-2)是一种锌离子依赖的蛋白水解酶,它们的蛋白表达与食管癌(esophageal carcinoma,EC)的发生、发展密切相关。本研究旨在探讨DcR3和MMP-2在食管癌组织中的表达、相互关系及与患者生存的相关性。方法:收集98例有完整临床病理资料和随访资料的食管癌标本及20例癌旁正常食管组织标本作为对照,采用EnVisionTM免疫组化法分别检测DcR3和MMP-2的表达,并电话随访所有患者的术后生存时间,分析其与患者各项临床指标及生存的关系。结果:98例食管癌组织中DcR3阳性表达64例(65.3%),阴性表达34例(34.7%);MMP-2阳性表达57例(58.2%),阴性表达41例(41.8%);DcR3、MMP-2在食管癌组织中的表达均显著高于癌旁正常食管组织(P<0.05);食管癌组织中DcR3、MMP-2表达均与肿瘤的浸润深度、淋巴结有无转移、临床分期及3年生存率密切相关(P<0.05);DcR3和MMP-2在食管癌组织中表达呈正相关(0<r<1,P<0.05);Kaplan-Meier生存曲线分析显示,患者术后生存率随DcR3、MMP-2阳性表达的增加呈下降趋势。结论:DcR3、MMP-2过表达与食管癌细胞恶性生物学行为及死亡率呈正相关,检测DcR3和MMP-2表达水平可为临床提供选择化疗方案的参考依据。 Background and purpose:DcR3 has been identified as a tumor necrosis factor receptor and MMP-2 has been identified as a Zinc-dependent protein hydrolase.Both DcR3 and MMP-2 have been identified to be associated with the survival of esophageal carcinoma.The aim of this study was to investigate the expression,relationship and correlation with patient survival with DcR3 and MMP-2 in patients with esophageal carcinoma.Methods:EnVisionTM immunohistochemistry method was used to examine the level of expression on the esophageal carcinoma cell decoy receptor 3(DcR3) and the matrix metalloproteinase-2(MMP-2) in all 98 surgical cases,each of them have their own follow-up data.A clinical pathologic analysis was also conducted.We followed up the survival time of all patients after surgery by telephone.Results:A DcR3 positive expression was found in 64/98(65.3%) of cases with esophageal carcinoma with a negative expression in 34/98(34.7%) cases,respectively.MMP-2 positive expression was found in 57/98(58.2%) of cases with esophageal carcinoma and negative expression was in 41/98(41.8%) cases,respectively.DcR3 positive expression was found in 27 cases(49.1%) in the group with more than three years survival rate,and those who died within 3 years had DcR3 positive expression were found in 37 cases(86.0%),respectively.There were significant difference between the two groups(P0.01).The MMP-2 positive expression was found in 26 cases(47.3%) in the group with more than 3 year survival rate but for the patients who died within three years,the MMP-2 expression was found positive in 31 cases(72.1%).There were also significant difference between the two groups(P0.05).Tumor differentiation degree,the lymph node metastasis,tumor invasion deep degree and clinical stage were significant prognostic factors in the survival group,when compared to the non-survivors(P0.05).DcR3 and MMP-2 were positively correlated between in the two groups(0r1).Conclusion:The DcR3 and MMP-2 over expression was positively related to cancer cell’s malignant behavior and death rate.It could serve as a reference for selecting chemotherapy protocal.The tumor differentiation degree,invasion degree,lymph node metastasis and its clinical stage were all important factors for patient survival.
