应用去抗原动物膜组织重建结膜的实验研究

摘要 目的探讨应用去抗原动物膜组织重建结膜的可行性。方法自制去抗原动物膜组织及保存羊膜,将兔上方球结膜用碱烧伤后制造大片结膜缺损模型,分别植入2种膜片,实验组(5只兔)植入眼表生物膜,对照组(5只兔)植入保存羊膜。术后观察大体情况、结膜愈合情况、材料外观与缝合强度,术后8周取创面区结膜组织行HE染色,做病理组织学检查。结果实验组兔结膜愈合情况良好,植片无感染、排异等并发症;病理组织学显示创面区组织结构与正常结膜相近,再生组织有3~4层鳞状上皮,植片的胶原未完全降解;缝合强度(2.65±0.48)N。对照组植入的保存羊膜在术后3周完全脱落,结膜愈合欠佳;病理组织学显示创面区结膜上皮生长情况欠佳,上皮下较多炎性细胞浸润;缝合强度(0.04±0.01)N。结论去抗原动物膜组织具有较强生物活性和低抗原性的特点,可作为结膜替代材料。 Objective To investigate the feasibility of conjunctival reconstruction by antigen-extracted porcine membrane.Methods The self-made antigen-extracted porcine membrane and preserved amniotic membrane were prepared as conjunctival substitutes.Then the most defect conjunctive models were established through alkali burning the rabbit upper bulbar conjunctiva,the porcine membranes were transplanted into eyes of the experimental group(5 cases)and the preserved amniotic membrane into the eyes of the control group(5 cases).The whole conditions,wound healing and substitute appearance and suturing intensity were observed after operation,the conjunctival within tissue around the wound area was selected for HE staining and histopathological examination at 8 weeks after operation.Results The conjunctive wound healing was good without infection and rejection in the experimental group.The histopathology showed the structure of wounding tissue was similar to the normal conjunctiva,the tabular epithelium covered in regenerated tissue was from 3 layers to 4 layers,and the grafts were degraded incompletely and the suturing intensity was(2.65±0.48)N.While the preserved amniotic membrane in the control group was completely rejected at 3 weeks after operation and the conjunctival healing was bad,the histopathological examination showed the growth of conjunctiva around the wound was bad,the inflammatory cells infiltrated into the sub-epithelium and the suturing intensity was(0.04±0.01)N.Conclusions Antigen-extracted porcine membrane has the characteristics of bioactivity and low antigenicity.It can be used as conjunctival substitute.
