时间分辨荧光免疫技术在乙肝两对半测定中的应用 被引量:4

摘要 目的比较时间分辨荧光免疫分析技术(TRFIA)和酶联免疫吸附试验(ELISA)对乙肝两对半的测定结果,探讨TRFIA在乙肝两对半测定中的应用价值。方法采用TRFIA对其配套两对半试剂盒进行准确性和重复性测试。对235例临床标本采用TRFIA和ELISA2种方法同时测定乙肝两对半,并对结果进行分析。结果 TRFIA试剂盒测定乙肝五项指标具有良好的准确性和重复性。与ELISA相比,TRFIA在HbsAg与HBeAg的测定上差异无统计学意义(P>0.05),而在HbsAb、HbeAb及HBcAb的测定上差异均有统计学意义(P<0.01)。在两对半模式的测定上,TRFIA与ELISA相比在HbsAb、HbeAb、HbcAb模式的检出率显著增高(P<0.05),其他医学模式相比差异均无统计学意义(P>0.05)。结论 TRFIA较ELISA特异性强、灵敏度高,是一种比较理想的定量检测方法。TRFIA通过乙肝两对半的定量测定为乙肝诊断、治疗及预后提供全面动态监测,并为疫苗接种提供可靠依据。 Objective To investigate the clinical value of time-resolved fluorescence immunoassay( TRFIA) on detec-ting five makers of hepatitis B by comparing the results of TRFIA and enzyme-linked immunoassay assay( ELISA) on detection of HBV markers. Methods The accuracy and the repeatability for the necessary reagent box was measured by TRFIA. Five markers of hepatitis B of 235 serum samples were measured by TRFIA and ELISA simultaneously. Results Five makers of hepatitis B was measured with good accuracy and repeatability by TRFIA. There was no remarkable significance between ELISA and TRFIA in detecting HBsAg and HBeAg( P 0. 05). But there was significant difference between ELISA and TRFIA in detecting HbsAb、HbeAb and HBcAb( P 0. 01). Compared with ELISA in HBV-M determination,the detection ratio of the mod-el( HbsAb、HbeAb、HbcAb) by TRFIA were significantly higher( P 0. 05) ,but others had no significant difference( P 0. 05). Conclusion TRFIA is more sensitive and higher specific than ELISA. It is an ideal quantitative detection method. TR-FIA can provide comprehensive and dynamic monitoring for the diagnosis、treatment and prognosis of hepatitis B thought the detection of HBV markers,and it also can provide reliable basis for vaccination.
