不同自然病程下肢深静脉血栓血流动力学分析(附203例报告)

摘要 目的:分析不同自然病程阶段下肢深静脉血栓血流动力学特点,为临床治疗方法选择提供依据。方法:回顾分析自2002年6月至2009年3月昆明医学院第一附属医院收治203例下肢深静脉血栓的临床资料,根据自然病程长短将其分为急性、亚急性和慢性3组。所有病例入院时均经彩色多普勒确诊并观察股、腘静脉流速峰值、内径等血流动力学参数。结果:除腘静脉内径在不同组间无差异外,股静脉流速峰值、腘静脉流速峰值、股静脉内径在组间具有显著统计学意义(P〈0.05)。结论:随自然病程的延续,下肢深静脉血栓的血流动力学会得到一定程度的改善,以代偿静脉梗阻所导致的静脉高压。 Objective To analyze the haemodynamic features of different natural-course lower extremity deep venous thrombosis (DVT), with intent to offering clinical proofs for DVT treatment selection. Methods A retrospective analysis has been performed from 203 clinically confirmed cases admitted between June 2002 and March 2009, which were categorized into 3 groups (acute, sub-acute and chronic)based on their natural courses. Flow velocities, diameter of femoral and popliteal vein were measured via Ultrasonography. Results Popliteal vein diameter was identical among 3 groups (acute, sub-acute and chronic). Femoral and popliteal vein velocity presented an ascending inclination, while femoral vein diameter presented a descending trend. Conclusion With its natural course progressing, haemodynamic state of lower extremity DVT will be improved to some extend to compensate for venous hypertension due to venous obstruction.
