摘要
1例55a男性患者,因下咽癌综合治疗后肝转移,静脉输入长春瑞滨注射液40mg+0.9%氯化钠注射液100mL,滴速80滴·min-1。滴完后10min左右患者出现吸气性呼吸困难,伴喉鸣音,全身湿冷,BP189/105mmHg,HR129次·min-1,R28次·min-1,SPO293%～81%。5min后出现神志不清,R12次·min-1,HR45次·min-1,SPO259%,血压为0。立即给予地塞米松10mg静注,吸痰,给氧5L·min-1。行气管插管给予简易呼吸仪器助通气时,患者出现心跳、呼吸骤停,立即给予盐酸肾上腺素1mg皮下注射,胸外心脏按压,1min后患者呼吸、心跳恢复,再经约1h的抢救和治疗,患者神志清醒并逐渐恢复。
A 55-year-old male patient with liver metastases after combined treatment for hypopharyngeal cancer received 40 mg of vinorelbine mixed in 100 mL of 0.9% sodium chloride injection by intravenous injection with the speed of 80 drops per minute.About 10 minutes after infusion,the patient developed inspiratory dyspnea,general clamminess,with blood pressure of 189/105 mmHg,heart rate of 129 beats per minute,respiratory rate of 28 breaths per minute,and SPO2 was 93%-81%.Five minutes later,he appeared obnubilation with respiratory rate of 12 breaths per minute,heart rate of 45 beats per minute,and zero blood pressure.Then such treatments as 10 mg of dexamethasone by intravenous infusion,aspiration of sputum,oxygen uptake with the speed of 5 L·min-1 were given immediately.When ventilating the patient with endotracheal intubation by using simple breathing apparatus,cardiac and breath arrest appeared.The patient received 1 mg of hydrochloride epinephrine by subcutaneous injection with external chest compression immediately.His breathing and heart rate recovered one minute later.The patient regained consciousness gradually one hour after salvage treatment.
