摘要
目的探讨胃平滑肌肉瘤的影像学表现及鉴别诊断。材料与方法回顾性分析经手术病理或／和活检证实的２１例胃平滑肌肉瘤的影像学表现。结果单发１９例．多发２例。Ｘ线片上腔内型１０例，表现为圆形或半圆形充盈缺损，肿瘤中心出现大而不规则的龛影，腔外型７例，为胃轮廓外压性改变及较大龛影。混合型４例，兼有上述二型表现。ＣＴ上表现为直径为６～１６ｃｍ分叶状肿块，密度不均匀且有不均匀强化，可有邻近侵犯和远处转移。结论胃低张双重造影和ＣＴ扫描密切结合，对提高胃平滑肌肉瘤的诊断和鉴别诊断很有帮助。
Purpose To study the imagiologic feature and differential diagnosis of gastric leiomyosarcoma. Materials and methods Imagiologic feature of gastric leiomyosarcoma which confirmed by operation,pathology and biopsy in 21 cases were analyzed retrospectively. results Single gastric leiomyosarcoma in 19 cases,multiple in 2 cases. x- ray feature was as following: Intraluminal type (10 cases)had a filling defect of round or oval shape and a larger,irregular ulcerative crater in the center of tumor extraluminal type (7 cases):the gastric contour had extrapressure deformity,displace and gaint crater. mixed type (4 cases)had two of above. CT showed exophytic growth pattern, lobulated contour with 6 -- 16cm diameter, heterogeneous enhancement,direct invasion and distant metastasis. Conclusion Hypotonic double contrast examination combined with CT scan,it is very helpful to improve the diagnostic and differential ability of gastric leiomyosarcoma.
