摘要
目的 研究翼状胬肉侵入角膜内不同程度对屈光状态的影响及手术前后眼屈光状态的改变.方法 初发翼状胬肉病人74例(80眼),均行翼状胬肉切除联合自体角膜缘干细胞移植术,检查术眼手术前后裸眼视力、最佳矫正视力及散光度,并按胬肉侵入角膜内程度分为≤2.5 mm 组、2.6~4.0 mm组、>4.0 mm组,比较术前翼状胬肉侵入角膜内不同程度对屈光状态的影响及手术前后屈光状态的改变.结果 术前3组平均裸眼视力、平均最佳矫正视力及平均散光度比较,差异有显著性(F=72.994~322.304,q=7.463~35.450,P<0.01).翼状胬肉侵入角膜内程度与角膜散光度呈正相关(r=0.833,P<0.01).3组术前、术后平均裸眼视力、平均最佳矫正视力、平均散光度比较,差异均有显著性(t=2.024~19.068,P〈0.05、0.01).结论 翼状胬肉侵入角膜内越多对角膜屈光状态的影响越大,手术治疗可使角膜散光得以改善,提高视力.
Objective To study the impact of different extent of invasion of pterygium upon cornea on refraction and its post-operative changes. Methods Seventy-four patients with primary pterygium (80 eyes) underwent pterygium excision combined with auto[ogous limbal stem cell transplantation. Pre and post-operative naked vision, the optimized corrected vision, and astigmatism of eyes operated were measured, and divided into three groups, ≤2.5 mm, 2.6--4.0 ram and 〉4.0 mm group, according to the extent of cornea affected. The post-operative results were compared among the three groups. Results Preoperatively, the differences of average naked vision, corrected vision and astigmatism among the three groups were significant (F= 72. 994--322. 304, q=7. 463--35. 450,P〈0.01). The degree of pterygium invasion in cornea was positively correlated with astigmatism (r= 0. 833, P〈0.01). A comparison between before and after surgery, the differences of average naked vision, optimized corrected vision and astigmatic degree were significant (t=2. 024-9. 068;P〈0.05, 0.01). Conclusion The more the pterygium intrudes into cornea, the more severe impact on the refraction would be. Surgical therapy may improve the refraction and raise the vision.
