摘要
目的探讨胆汁CA19-9在胆道良恶性病变的诊断价值。方法本研究选择经手术及病理证实的胆道恶性肿瘤26例,胆道良性病变92例,正常健康人13例,采用RIA同时联合检测胆汁CA19-9、CEA,进行定量分析。结果胆汁CA19-9的含量分界值为12000u/ml,其敏感性,特异性分别为92.31%,72.83%;胆汁CEA水平分界值为490ng/ml,其相应指标分别为57.69%、78.26%。胆汁CA19-9、CEA含量的假阳性率分别为27.17%、21.73%。结论胆汁CA19-9是诊断胆道癌,判断胆道癌是否被彻底切除,以及监测该病治疗反应有效的肿瘤标志物。
Objective To evaluate the lever of bile CA199 in diagnosing biliary tract
carcinoma. Methods We assayed the content of bile CA199CEA in 26 cases with malignant
biliary tract neoplasm and 92 cases with benign biliary lesion (BBL)which are all proved both
operatively and physiologically,and 13 healthy persons.Radioimmunoassy (RIA) was used to
quanititative analyisis the content of CA199 and CEA in bile for patients with benign biliary
lesion(BBL) or with biliary tract carinoma(BIC) before and after operation. Results The average
content of bile CA199 in BTC,BBL patients and normal persons are 15000u/ml,6000u/ml and
5000u/ml respectively.The dividing value of bile CA199 is 12000u/ml,and its
sensitivity,specificity are 92.31% and 72.38% respectively.The dividing value of bile CEA is
490ng/ml,and its corresponding indexes are 57.69% and 78.26%.The FPR(false positive rate)of
bile CA199,CEA content are 27.17% and 21.74%. ConclusionZCA199 is a effective tumor marker
in diagnosing and indicating operation,Carcinoembryonic antigenas well as supervising the
therapeutic feedback about the disease.
