摘要
目的探讨不同碘摄入水平对孕妇甲状腺功能的影响。方法采用ACS-180化学发光分析仪检测837名孕妇的血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺激素(FT4)、促甲状腺激素(TSH)和人绒毛膜促性腺激素(hCG);利用碘催化砷铈反应原理,采用冷消解快速尿碘定量检测方法测定孕妇1次随意尿的尿碘含量。结果碘摄入适宜组、碘摄入不足组和碘摄入过量组的甲状腺功能紊乱发生率分别为6.5%、9.1%和9.1%,3组发生率差异无统计学意义(P>0.05)。3组不同碘摄入水平的甲状腺功能紊乱均以亚临床甲状腺功能减低居多,3组间发生率的差异无统计学意义(P>0.05);碘摄入过量组FT3和hCG低于碘摄入适宜组(P<0.05),2组的FT4和TSH差异无统计学意义(P>0.05);碘摄入不足组的FT4高于碘摄入适宜组(P<0.05),2组的FT3、TSH和hCG差异无统计学意义(P>0.05)。结论不同碘摄入水平孕妇的甲状腺功能紊乱的发生与其甲状腺激素代偿性变化无关,可能主要是与妊娠期母体免疫状态的改变有关。
Objective To explore the effects of different iodine intake levels on thyroid function of pregnant women.Methods FT3,FT4,TSH and hCG were measured in 837 pregnancy by ACS-180 automatic chemiluninescence analyzer.The Urinary iodine were measured in a random urine by the use of iodine catalytic reaction mechanism of Arsenic-Cerium,rapid urinary iodine was measured using cold digestion method of quantitative detection.Results The incidence of thyroid dysfunction in the pregnancy of appropriate iodine intake,insufficient iodine intake and excessive iodine intake were 6.5%,9.1% and 9.1% respectively.There were no significant differences in the three group(P〉0.05).The incidence of subclinical hyperthyroidism was more in various iodine intake group.No significant differences were found in them(P〉0.05).FT3 and hCG in the pregnancy of excessive iodine intake were lower than that of appropriate iodine intake(P〈0.05).There were no significant differences in FT4 and TSH between the two groups(P〉0.05).FT4 in the pregnancy of insufficient iodine intake was higher than that of appropriate iodine intake(P〈0.05).There were no significant differences in FT3,TSH and hCG between the two groups(P〉0.05).Conclusion The incidence of pregnancy thyroid dysfunction in different iodine intake levels was independent of compensatory changes of thyroid hormone,but may depends on the changes in maternal immune status.
