摘要
目的:探讨2型糖尿病(T2DM)伴自主神经病变(AN)(T2DAN)患者氨基末端脑钠素前体(NT-ProBNP)的变化情况,阐明NT-ProBNP与T2DAN的相关关系。方法:单纯T2DM组患者40例,T2DAN组患者35例,另选取本院体检中心健康体检者21例作为对照组,应用酶联免疫吸附实验(ELISA)检测各组患者血浆中NT-ProBNP水平。结果:T2DAN组NT-ProBNP水平明显高于T2DM组及对照组(均P<0.01)。NT-ProBNP与病程、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、平卧位心率(HR)及立卧位收缩压(SBP)变化呈正相关关系,相关系数(r)分别为0.957、0.932、0.912、0.876及0.924(P<0.01),与空腹血清C-肽(FC-P)、深呼吸心率变化(HRV)、立卧位HRV、30∶15比值及乏氏指数呈负相关关系,相关系数(r)分别为-0.755、-0.766、-0.698、-0.732及-0.812(P<0.01)。结论:在T2DAN患者中,血浆NT-ProBNP过度表达与T2DAN有明确相关性。
Objective To study the changes of N-terminal pro brain natriuretic peptide （NT-ProBNP）in patients with type 2diabetes mellitus（T2DM）complicated with autonomic neuropathy（AN）（T2DAN）,and illustrate the correlation between NT-ProBNP and T2DAN.Methods 40patients with simple T2DM,35patients with T2DAN were selected,and 21healthy subjects were used as control group.The expressions of NT-ProBNP in the peripheral blood of the patients and healthy subjects were detected with ELISA.Results The levels of NT-ProBNP in T2DAN group were higher than those in T2DM group（P0.01）and healthy control group（P0.01）.NT-ProBNP was positively correlated with course,FPG and HbA1c,HR in horizontal position,change of SBP from supine to standing（r=0.957,P0.01;r=0.932,P0.01;r=0.912,r=0.876,P0.01;r=0.924,P0.01）,but negatively correlated with FC-P,HRV with deep respiration,HRV with postural change,30：15ratio,valsalva ratio（r=-0.755,P0.01;r=-0.766,P0.01;r=-0.698,P0.01;;r=-0.732,P0.01;r=-0.812,P0.01）.Conclusion NT-ProBNP over-expresses and has a clear correlation with DAN in patients with T2DAN.
