移位性跟骨关节内骨折手术与非手术治疗的疗效比较 被引量:6

摘要 目的 比较切开复位钢板内固定术与非手术治疗移位性跟骨关节内骨折的临床疗效.方法 对2002年2月至2008年11月诊治并获得随访的114例(128足)移位性跟骨关节内骨折患者进行回顾性分析,采用切开复位钢板内固定治疗(手术治疗组)62例(70足),男58例(65足),女4例(5足);年龄21~69岁,平均40岁.采用保守治疗(非手术治疗)52例(58足),男49例(54足),女3例(4足);年龄19~68岁,平均38岁.根据手术前、后及随访时X线片对所有患者跟骨B(o)hler角、Gissane角及跟骨高度、宽度、长度进行评估,采用Maryland足评分系统评价患足功能.结果 114例患者术后获12~48个月(平均28个月)随访.手术治疗组X线片示术后跟骨B(o)hler角、Gissane角及跟骨高度、宽度、长度均获得满意矫正,术后未发现明显丢失;术后早期出现切口裂开、坏死及感染12足,占17.1%(12/70);Maryland足部评分显示:优38足,良24足,可8足,优良率为88.6%.非手术治疗组伤后随访跟骨B(o)hler角、Gissane 角及跟骨高度、宽度出现进一步丢失,Maryland足部评分显示:优11足,良17足,可20足,差10足,优良率为48.3%.手术治疗组疗效明显优于非手术治疗组,差异有统计学意义(P<0.05).结论 切开复位钢板内固定术是治疗移位性跟骨关节内骨折的有效方法,虽然术后切口并发症难以完全避免,但其临床疗效明显优于非手术治疗.
