经颅超声检查在帕金森病诊断中的应用 被引量:9

摘要 目的探讨经颅超声检查(TCS)诊断帕金森病(PD)的临床应用价值。方法对80例PD患者(PD组)及80名健康志愿者(对照组)进行TCS检查,根据黑质回声区域的强度分为Ⅰ~Ⅴ级。单侧黑质强回声≥Ⅲ级、面积>0.20cm2且双侧黑质强回声总面积/中脑总面积(S/M)比值≥7%提示PD可能。结果 PD组异常黑质强回声者58例(58/80,72.50%),明显高于对照组的10名(10/80,12.50%),差异有统计学意义(P<0.01);双侧黑质强回声总面积的中位数、四分位数间距分别为0.46cm2、0.69cm2,S/M比值的中位数、四分位数间距分别为10.00、14.00,明显高于对照组(P均<0.01)。TCS诊断PD的敏感度和特异度分别为72.50%、87.50%。结论 TCS对PD的诊断具有一定价值,但少数老年患者由于颞窗透声不良而不适合此项检查。 Objective To assess the clinical value of transcranial sonography (TCS) in diagnosis of Parkinson's disease (PD). Methods Eighty PD patients (PD group) and 80 normal people (control group) underwent TCS. The echo singal intensity of the nigra was classified into Ⅰ—Ⅴ as half quantitative analysis. When the echo singal intensity of the substantia nigra ≥Ⅲ,the echo enhanced area 0.20 cm2 and the ratio of substantia nigra hyperechogenicity to the area of the whole midbrain (S/M) ≥7%,the patient was thought to be with PD. Results TCS detected more patients (58/80,72.50%) with substantia nigra hyperechogenicity in PD group than those in control group (10/80,12.50%). In PD group,the median and the quartile of the area of bilateral substantia nigar hyperechogenicity was 0.46 cm2 and 0.69 cm2,those of S/M was 10.00 and 14.00,respectively,greater than those of control group (both P0.01). The sensitivity and the speciality of TCS on diagnosis of PD was 72.50% and 87.50%,respectively. Conclusion According to the change of nigra echogenicity,TCS may provide some valuable information for clinical diagnosis of PD,the main limiting factor is that some individuals are unsuitable to the exam for their unadequated temporale bone windows.
