卵巢间质动脉血流预测卵巢储备功能及超排卵反应性的研究 被引量:15

摘要 目的:探讨卵巢间质动脉血流超声学指标预测卵巢储备功能及超排卵反应性的可靠性。方法:经阴道彩色B型超声检测164例月经第2天(M2)卵巢间质动脉血流搏动指数(PI)、阻力指数(RI)、收缩期峰值流速(S)/舒张期低值流速(D),预测卵巢储备功能和超排卵反应性,并与用基础FSH值+窦卵泡数+卵巢体积的传统预测方法进行比较;用回收卵母细胞数回顾判断卵巢间质动脉血流指标预测卵巢储备功能及超排卵反应性的可靠性。结果:卵巢储备功能正常组与异常组比较,M2的PI(P<0.05))、RI(P<0.05)有统计学意义;超排卵反应良好组与反应不良组比较,两组M2,PI,RI,S/D(P<0.05)有差异。用卵巢间质动脉血流超声指标预测卵巢储备功能,与传统判断卵巢储备功能结果相一致,预测的可靠性为78.66%(129/164);卵巢基础间质动脉血流预测卵巢反应性的可靠性为76.22%(125/164)。结论:卵巢间质动脉血流PI、RI和S/D可用于预测卵巢储备功能及超排卵反应性。 Objective:To explore the reliability on prediction of ovarian reserve and ovarian response by ovarian artery blood flow. Methods:The ovarian artery blood flow ,which include S, D, PI, RI, and S/D ,were detected by transvaginal color Doppler. Those ultrasound guidelines were used to predict ovarian reserve and COH, and compared with traditional method by FSH basal level + perifollicular numbers +ovarian volume。In IVF/ICSI-ET cycle, ovarian reserve and COH were valued ultimately by oocytes to retrieve and maturation oocytes. Results:It can be gained the same effect that ovarian reserve was evaluated by the testing of ovarian artery blood flow and FSH basal level + perifollicular numbers +ovarian volume. PI, RI was significantly different between the normal ovary reserve and the anormal ovary reserve(P0.05 and P0.05).There is different that the group of normal COH was compared with the group of anormal COH about PI(P=0.023),RI(P0.05),S/D(P0.05). The data show that it was 78.66%(129/164) that the reliability of predicting ovarian reserve by ovarian artery blood flow,and it was 76.22%(125/164) that the reliability of predicting COH. Conclusion:This study suggests that PI, RI and S/D are the major guidelines to predict ovarian reserve and ovarian response.
