血必净联合无创通气治疗慢性阻塞性肺疾病合并重症型呼吸衰竭的临床分析 被引量:8

Combination of Xuebijing and Non-invasive Mechanical Ventilation Treatment for Chronic Obstructive Pulmonary Disease Patients with Severe Respiratory Failure

摘要 目的:探讨血必净及双水平气道正压(BiPAP)通气在慢性阻塞性肺疾病(COPD)合并重症型呼吸衰竭治疗中的作用。方法:回顾分析65例接受BiPAP治疗的COPD合并重症型呼吸衰竭患者的临床资料。所有患者随机分为2组:治疗组(n=35)给予血必净及无创通气加常规治疗;对照组(n=30)给予无创通气加常规治疗。检测2组治疗前后血常规、动脉血气、C反应蛋白(CRP)。结果:治疗组较对照组症状、体征、血PaO2、PaCO2、pH、CRP改善更明显(P<0.05),治疗组有效率高于对照组(P<0.05)。结论:血必净配合无创通气对COPD合并重症型呼吸衰竭抢救治疗是一种有效的措施及方案。 OBJECTIVE: To evaluate the clinical efficacy of combination of Xuebijing and non-invasive mechanical ventilation treatment for chronic obstructive pulmonary disease (COPD) patients with severe respiratory failure. METHODS: The data of 65 COPD patients with severe respiratory failure receiving intermittent bi-level positive airway pressure (BiPAP) ventilation were analyzed retrospectively. All the patients were randomly divided into 2 groups: control group(n=30) receiving BiPAP and conventional treatment and Xuebijing group(n=35) receiving Xuebijing, BiPAP and conventional treatment. PaO2,PaCO2, pH and CRP were detected in two groups. RESULTS: Compared with control group, Xuebijing group showed more improvement in PaO2, PaCO2, pH, symptoms, signs and white blood cell count (P〈0.05). Effective rate of Xuebijing group was higher than control group (P〈0.05). CONCLUSION: Combination of Xuebijing and non-invasive mechanical ventilation treatment is an effective therapeutic regimen for COPD patients with severe respiratory failure.
