颅脑创伤合并四肢骨折患者早期骨折复位的生长激素变化研究 被引量:4

摘要 目的动态观察颅脑创伤合并四肢骨折患者血清生长激素(GH)水平,探讨骨折手术复位时机的选择。方法选取单纯性颅脑创伤患者23例和颅脑创伤合并四肢骨折患者22例,分别于创伤后1d、3d、7d、14d测定血清GH水平。比较两组各时间点GH水平,两组各时间点GH升高患者的构成比及伤后14d内GH升高患者构成比。结果伤后单纯颅脑创伤组和颅脑创伤合并四肢骨折组患者各时间点GH升高发生率分别为13.64%~36.36%和21.74%~30.43%;伤后14d内两组患者出现过GH升高的发生率分别为52.17%和59.09%,两组各时间点GH水平差异无统计学意义(P>0.05),GH升高的发生率差异均无统计学意义(P>0.05)。结论颅脑创伤急性期患者GH水平升高;结合文献报道,对合并有手术指征的四肢骨折患者应尽早行骨折手术复位,依据GH升高提出颅脑创伤合并骨折需手术复位的手术"窗口期"。
