摘要
目的观察2型糖尿病肾病(DN)不同阶段血清脂联素(ADPN)的变化趋势,探讨血清ADPN与影响因素的关系。方法选择30例健康体检者作为对照组,89例DN患者根据尿白蛋白排泄率分为三组:正常白蛋白尿组(35例);微量白蛋白尿组(32例);大量白蛋白尿组(22例)。检测血清ADPN(ELISA法),同时测定BMI、WHR、HbAlc、血脂水平、尿白蛋白排泄率等指标,比较各组间的差异。结果 (1)DN患者血清ADPN水平低于正常人群;DN患者各组间比较,大量白蛋白尿组的血清ADPN水平最高,差异具有统计学意义(P<0.05)。(2)多元逐步回归分析显示:BMI、病程、UAER、DBP、2hPG、HbA1c与脂联素水平密切相关。(3)相关分析提示:血清ADPN与BMI、WHR、HbA1c、FPG、2hPG呈负相关,与年龄、病程、UAER呈正相关。结论 (1)DN患者血清ADPN水平低于正常人群。其血清ADPN水平与年龄、病程、UAER、HDL呈正相关,与血糖控制状态(FPG、2hPG和HbA1c)、BMI、WHR呈现负相关关系。(2)DN患者血清脂联素随着尿蛋白排泄率增加而逐步升高。
Objective To investigate the serum adiponectin in different stages of diabetic nephropathy and analyze the relationship among factors as follows：urinary albumin excretion rate（UAER）,blood-lipide,body mass in-dex,glycated hemoglobin and serum adiponectin.Methods 89 type-2 diabetic patients with nephropathy were included in the present study,and 30 healthy people acted as the control.According UAER,the diabetic patients were divided into three groups：normal albuminura（35 cases,UAER30 mg/l/24h）,microalbuminura（32 cases,UAER 30-300 mg/24h）and macroalbuminura group（22 cases,UAER≥300 mg/24h）.Serum adiponectin was measured by ELISA,and body mass index（BMI）,waist hip radio（WHR）,HbAlc,blood lipid and UAER were measured and compared.Results 1.The level of serum adiponectin was lower in diabetic nephropathy patients than that in normal control（P0.05）.Among diabetic patients,the serum adiponectin in macroalbuminura group was significantly higher than that in normal and microalbuminura groups（P0.05）.2.Serum adiponectin was positively associated with age,course and UAER;negatively associated with BMI,WHR,HbA1c,FPG and 2hPG.Conclusion The level of serum adiponectin is lower in diabetic nephropathy,and positively associated with age,course and UAER;negatively associated with the control state of blood.
