摘要
目的：观察三联疗法根除幽门螺杆菌（ＨＰ），预防溃疡复发与再出血的远期疗效。方法：选择有ＨＰ感染的出血性十二指肠溃疡患者５８ 例，随机分为两组。治疗组３６ 例，用丽珠得乐冲剂、呋喃唑酮片剂、泰胃美片治疗２周；对照组２２例，单用泰胃美片治疗２周。疗程结束后，两组患者均再用泰胃美治疗２ 周，总疗程结束后４ 周及每半年复查胃镜和ＨＰ，定期随访２ 年。结果：治疗组和对照组溃疡愈合率分别为１００％ 和８１．８％ ，ＨＰ根除率分别为８０．５％ 和９．１％ 。随访２ 年后，治疗组溃疡复发率为１３．９％ ，再出血率为５．５％ ，对照组溃疡复发率为９０．９％ ，再出血率为４５．４％ ，两组溃疡复发率与再出血率比较有显著性差异（ Ｐ＜ ０．０５）。结论：三联疗法具有较好的根除ＨＰ、明显减少溃疡复发和再出血发生的作用。
Objective: To observe the long term effect of triple therapy on HP relative bleeding duodenal ulcer and HP eradication ratio. Methods:58 patients with HP positive and bleeding duodenal ulcer examined by endoscopy and test were randomly divided into two groups. 36 patients(treatment group)were treated with Colloidal bismuth subcitrale(110 mg, qid), furazolidone tablels(0.1 qid) for two weeks,Tagament tablets (800 mg, qd) for two weeks.22 patients(control group) with only Tagament tablets (800 mg, qd)for two weeks. After these treatments all the 58 patients were treated with Tagament for two weeks.And four weeks later endoscope and checking of HP were performed in the 58 patients and reexamined every other six months.All cases were followed up for two years.Results:Ulcer healing rates in treatmeht group and control group were 100% 81.8%,respectively. HP eradication rates in treatmemt group and control group were 80.5%,9.1%,respectively. Two years later,the recurrence and rebleeding rate was 13.9% and 5.5% in treatment group,90.9% and 45.4% in control group.The difference between control group and treatment goup and group was very significant(P< 0.05 ).Conclusion:The triple therapy can eradicated HP well and significantly reduce the rate of rebleeding and recurrence of duodenal ulcer.
