尿沉渣UF-100检查中鉴别女性尿路感染与尿液污染指标参数的探讨 被引量:9

摘要 目的通过对临床尿路感染组和尿液标本污染组患者尿沉渣和干化学结果对比分析,寻找鉴别尿常规标本是尿路感染或者白带污染的指标参数。方法用受试者工作特征曲线(receiver operating characteristic curve,ROC)分别对尿沉渣结果中白细胞(WBC)、上皮细胞数(EC)、上皮细胞/白细胞比值(E/W)进行评价并计算曲线下面积。结果 WBC、EC、E/W的ROC曲线下面积分别为0.209,0.746,0.976,E/W比值的曲线下面积最大,其最优截断点约为0.22,诊断的灵敏度为0.926,特异度为0.934。结论 EC和E/W可以作为鉴别感染组和污染组的临床检验指标参数,E/W比值对尿液标本污染的诊断价值最高,其最优截断点为0.22。 Objective To looking for indicators of identify urinary tract infection and urinary contamination with UF-100 examination of urinary sediment and dry chemical group.Methods the results of white blood cell count(WBC),epithelial cells(EC),epithelial cells white blood cell ratio(E/ W)were analysis and calculated the area under the receiver operating characteristic curve(ROC).Results WBC,EC,E/W in the area under the ROC curve were 0.209,0.746,0.976.E/W ratio have the largest area under the curve.The optimal truncation point is about 0.22 with the diagnostic sensitivity is 0.926 and specificity 0.934.Conclusion EC and E/W can be used as identification of infection and contamination,E/W ratio have the largest diagnostic value of urine contamination.The optimal cut point is 0.22.
