摘要
目的运用免疫细胞化学方法检测脑脊液单核细胞内结核分枝杆菌早期分泌抗原(ESAT-6),探讨其对结核性脑膜炎早期诊断的意义。方法选择31例结核性脑膜炎患者和37例非结核性脑膜炎患者,运用免疫细胞化学方法检测脑脊液单核ESAT-6抗原,同时进行脑脊液抗酸染色、结核抗体和腺苷脱氨酶(ADA)检测,然后比较各种方法的阳性率、敏感性和特异性。结果免疫细胞化学方法检测脑脊液ESAT-6(免疫优势抗原)抗原的阳性率较其它检测方法的阳性率显著增高(P<0.01),其ESAT-6抗原诊断结核性脑膜炎的敏感性和特异性明显高于脑脊液抗酸染色、结核抗体和ADA检测。结论检测脑脊液单核细胞内ESAT-6抗原对结核性脑膜炎的早期诊断具有重要价值。
Objective To detect the early secrete antigen 6(ESAT-6) of tuberculosis bacilli in cerebrospinal fluid mononuclear cells and to explore the method of early diagnosis for tuberculous meningitis.Methods 31 patients with tuberculosis meningitis and 37 cases of no tuberculous meningitis patients were selected.The immunocytology method was used to detect ESAT-6 of tuberculosis bacilli in CSF mononuclear cells,while acid fast stain of CSF,antibody of tuberculosis detection of CSF and ADA detection of CSF were performed.Then the positive rates of sensitivities and specificities were compared.Results The positive rate of ESAT-6 detecting was significantly higher than other detection methods(P0.01).The sensitivity and specificity were significantly higher than acid fast stain,tuberculosis antibody and ADA in CSF.Conclusion The detection of ESAT-6 of tuberculosis bacilli in CSF mononuclear cells by immunocytological method is very important for early diagnosis of tuberculous meningitis.
