摘要
目的探讨运用超声定位光散射乳腺成像(US-guided optical imaging system,OPTIMUS)系统对乳腺肿块的定性诊断价值。方法回顾分析经病理检查证实且同时应用OPTIMUS系统和彩色多普勒超声诊断的36例乳腺肿块,评价两种方法的诊断结果与病理诊断的一致性。并绘制受试者工作特征(receiver operating characteristic,ROC)曲线,计算曲线下面积(area under the curve,AUC),判断OPTIMUS系统及彩色多普勒超声两种检查手段的准确性。结果 36例乳腺肿块术后病理诊断良性16个,恶性20个。彩色多普勒超声诊断乳腺肿块中良性17个,恶性19个;OPTIMUS系统诊断乳腺肿块中良性14个,恶性22个。OPTIMUS系统、彩色多普勒超声诊断与病理诊断结果的一致性检验Kappa值分别为0.772、0.497,其两者之间一致性检验Kappa值为0.606。OPTIMUS系统、彩色多普勒超声诊断的AUC分别为0.923(95%CI:0.834~1.013)、0.802(95%CI:0.654~0.949)。结论 OPTIMUS对乳腺肿块的定性判断较彩色多普勒超声诊断准确性及敏感性高,两者结合可进一步提高诊断准确性。
Objective To study the value of diffused optical breast imaging with ultrasound(OPTIMUS system) in the diagnosis of breast tumors.Methods The images of 36 breast tumors by OPTIMUS system were reviewed and compared with the results of Doppler ultrasound and pathology.The receiver operating characteristic(ROC) curve was done to calculate the area under the curve and to evaluate the accuracy and the clinical value of OPTIMUS system in the diagnosis of breast tumors.Results In the 36 cases,pathological diagnosis showed 16 cases benign and 20 cases malignant.Doppler ultrasound diagnosed 17 cases benign and 19 cases malignant;OPTIMUS system diagnosis showed 14 cases benign and 22 cases malignant.Based on the pathological results,the consistency checking Kappa value was 0.772 and 0.497 for OPTIMUS system and Doppler ultrasound,respectively;and kappa was 0.606 between Doppler ultrasound and OPTIMUS system.The area under ROC for OPTIMUS system and Doppler ultrasound was 0.923(95%CI 0.834~1.013) and 0.802(95%CI 0.654~0.949).Conclusion OPTIMUS system is a non-invasive and highly effective diagnostic tool for breast tumors and it is well consistent with pathology.
