绝经后妇女肾虚证与骨密度及载脂蛋白E基因多态性的相关性研究

Study on Correlation of Kidney Deficiency Syndrome in Postmenopausal Women with Bone Mineral Density and Apolipoprotein E Gene Polymorphism

摘要 目的:探讨绝经后妇女肾虚证与骨密度(BMD)及载脂蛋白E(Apo E)基因多态性的内在联系。方法:以80例绝经后肾虚证妇女为肾虚组,另设65例绝经后妇女为对照组,同步检测BMD,并运用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)方法检测2组ApoE基因型和等位基因频率。结果:肾虚组与对照组BMD比较,肾虚组髋关节股骨颈(Neck)、股骨大转子(Troch)以及Wards三角(Ward)不同部位的BMD均较对照组显著降低,差异均有非常显著性意义(P<0.01)。而肾虚组各部位BMD的大小依次为Neck>Troch>Ward,BMD变化表现出了一定的规律。肾虚组检测到5种基因型,对照组未检测到ε_4/ε_4基因型;肾虚组ε_4等位基因频率显著高于对照组(P<0.05)。BMD与Apo E等位基因ε_4频率之间存在相关性:各部位BMD改变与ApoE等位基因ε_4频率呈正相关。结论:绝经后妇女肾虚证与BMD有直接相应变化的关系,BMD测定值可作为观察肾虚证治疗效果的参考依据。ApoE等位基因ε_4频率升高与绝经后妇女肾虚证相关,ApoE等位基因ε_4可能是肾虚证的危险因子。 Objective: To explore the correlation of kidney deficiency syndrome in postmenopausal women with bone mineral density(BMD)and apolipoprotein E(Apo E) gene polymorphism. Methods: Eighty postmenopausal women with kidney deficiency syndrome were enrolled into kidney deficiency(KD)group, and sixty-five postmenopausal women served as the control group. BMD was detected in two groups simultaneously. Apo E genotypes and allele frequencies were also detected by polymerase chain reaction - restriction fragment length polymorphism (PCR-RFLP) method. Results: There were significant differences (P〈 0.01) in BMD at hip femoral neck (Neck), greater trochanter (Troch) and Wards triangle (VVard) between KD group and the control group. The sequence of BMD in different parts of the femur was as follows: Neck〉Troch〉Ward. Five genotypes were detected in KD group, and ε4/ε4genotype was undetectable in the control group. The frequency of ε4 allele in KD group was higher than that in the control group (P 〈 0.05). Positive correlation was found in Apo E ε4 allele frequency with BMD at various parts. Conclusion: Kidney deficiency syndrome in postmenopausal women is correlated with BMD, which can be used as one of the response criteria of kidney deficiency syndrome. Kidney deficiency syndrome in postmenopausal women is also correlated with the increase of Apo Eε4 allele frequency, indicating that Apo E ε4 allele may be a risk factor of kidney deficiency syndrome.
