摘要
目的研制既有抗感染能力又有较高成骨活性的新型骨移植材料。方法用高效液相色谱法(HPLC)测定庆大霉素重组合异种骨(GRBX)在小鼠肌袋内药物释放度,并在污染的兔桡骨骨缺损模型中观察其抗感染与骨修复功能。结果明胶包裹4mg庆大霉素的GRBX具有良好的药物缓释功能,在植入部位周围软组织中维持有效浓度可达17天,提高明胶浓度能进一步延缓药物释放度。GRBX植入3×107CFU金葡菌污染的兔桡骨干1.5cm骨缺损模型可有效抑制感染,16周后获得完全骨连接。结论局部应用抗生素与骨移植相结合有可能改进开放性骨损伤及感染后骨不连骨缺损的治疗。
Objective\ To prepare a new kind of bone graft which has an antiinfective function and high osteogenesis at the same time, and to improve the treatment of infective bone nonunion and bone defect.Method\ The released concentration of gentamicin in mice muscle was determined in different time points by HPLC. The antiinfective function and bone repair function of gentamicin loaded reconstituted bone xenograft (G RBX) as a material for implantation in contaminated radial defect was investigated in a rabbit model.Results\ G RBX encapsulated 4 mg gentamicin had a better function of sustained release. Effective releasing of antibiotics from G RBX maintained 17 days. The releasing rate can be slowed down by increasing the concentration of gelatin. Primary implantation of G RBX into osseous defect contaminated with 3×10 7 CFU staphylococcus aureus could result in bone union without infection.Conclusion\ Local antibiotic supplementation of graft preparations may be useful for the treatment of infective bone nonunion and bone defect.
