摘要
目的通过分析藻酸盐敷料联合水凝胶在治疗咽喉肿瘤手术后咽瘘及颈深间隙感染后咽瘘的临床疗效,探讨并总结此类疾病的治疗方案及经验。方法对10例咽喉部恶性肿瘤术后及2例颈深间隙脓肿继发咽瘘患者,根据疾病进展情况进行分期,采用藻酸盐敷料联合水凝胶换药治疗,观察并分析其临床疗效。结果喉癌下咽癌术后咽瘘者愈合9例,未愈1例,治愈率90%。颈深部脓肿继发咽瘘者2例,全部愈合。喉癌下咽癌术后咽瘘者愈合时间最短5d,最长40d,平均26.7d。颈深部脓肿继发咽瘘者愈合的时间分别为45d和60d,平均52.5d。结论藻酸盐联合水凝胶敷料可以创造湿性愈合环境,在控制感染、加速创面愈合方面有良好效果,为咽瘘的治疗提供了一种有效的新方法。
Objective To evaluate the clinical effect of combination therapy with alginate dressing and hydrogel for pharyngocutaneous fistula caused by pharyngolaryngeal surgery and neck infection.To dissucss and summarize treatment experience of these diseases.Methods Ten fistula following pharyngolaryngeal surgery and 2 fistula following neck infection were studied.According to the feature of the wound,we classified them to three phases.Alginate dressing and hydrogel were applied to treat the fistula.Results In 10 cases of pharyngolaryngeal surgery,9 were cured,and the effective rate was 90%.The two cases of neck infection were both cured.Conclusion Alginate dressing and hydrogel combination therapy was proved effective and practical for pharyngocutaneous fistula treatment.
