AIF、Cyt-C、Apaf-1在冠状动脉介入治疗中水平变化的临床研究 被引量:2

摘要 目的通过测定冠心病患者介入治疗前后血清中凋亡诱导因子(AIF)、细胞色素C(Cyt-C)、凋亡蛋白酶活化因子(Apaf-1)水平的变化,探讨冠状动脉介入治疗(PCI)对冠心病患者血清中AIF、Cyt-C及Apaf-1水平的影响。方法冠心病介入治疗组(PCI组)36例和单纯冠脉造影组(CAG组)28例,在术前、术后3 h、6 h、12 h、24 h以及48h分别测定血清AIF、Cyt-C及Apaf-1水平。结果术前PCI组与CAG组的AIF、Cyt-C水平差异均有统计学意义,而术前PCI组与CAG组的Apaf-1水平差异无统计学意义。PCI组术后AIF、Cyt-C及Apaf-1水平较术前有升高趋势,在术后12 h达高峰,以后逐渐恢复至基线水平,但各时间点水平差异无统计学意义。CAG组术后血浆AIF、Cyt-C及Apaf-1水平较术前有升高趋势,至术后48 h达到峰值,AIF术后48 h水平与术前相比差异具有统计学意义(P<0.05)。PCI组与CAG组术后各时间点AIF、Cyt-C与Apaf-1水平相比均显著增高,于术后12 h AIF、Cyt-C与Apaf-1水平PCI和CAG两组之间的差异均具有统计学意义,且AIF术后6 h、24 h水平PCI和CAG两组之间的差异均具有统计学意义。结论 PCI可引起血AIF、Cyt-C及Apaf-1水平升高,PCI组术前AIF、Cyt-C及Apaf-1水平明显高于CAG组,提示冠状动脉病变程度与细胞凋亡呈正相关。冠脉介入治疗术后可能出现短暂性心肌缺血,加重了心肌损伤,AIF、Cyt-C及Apaf-1可作为细胞凋亡的监测指标并有助于冠心病患者PCI术后不典型心肌损伤的诊断。AIF可能是反映细胞损伤的敏感指标之一。 Objective By patients with coronary artery disease before and after interventional treatment in serum apoptosis-inducing factors(AIF),cytochrome C(C),Cyt-C apoptosis protease activated factor(Apaf-1) level changes,coronary interention(PCI) in patients with coronary artery disease,Cyt-C AIF serum and Apaf-1.Methods Perc-Utaneous coronary intervention(PCI) 36 cases and pure coronary angiography CAG group(group),28 cases in pre-operative and postoperative 3 h,6 h 12 h and 24 h,determination and 48 h AIF serum Cyt-C,Apaf-1 and level.Result Preoperative group and the group of PCI and CAG AIF,Cyt-C levels were statistically significant difference,while preoperative PCI group and the group Apaf-1 CAG level difference was statistically significant.PCI group,Cyt-C and AIF Apaf-1 level,had a preoperative 12h reach peak in postoperative recovery,later to baseline,but each time point level difference was statistically significant.AIF plasma CAG after group Cyt-c Apaf-1 and there was increased,preoperative and postoperative 48 h peak AIF 48h level compared with the preoperative after a statistically significant difference(P0.05).PCI groups and different time points CAG group,Cyt-C AIF Apaf-1 with all levels were sign-ificantly higher in surgery,Cyt-C12 h AIF,with Apaf-1 level CAG PCI and differences between the two groups was statistically significant,and postoperative 6 h AIF 24 h CAG PCI and level,differences between the two groups was sta-tistically significant.Conclusion The PCI can cause blood AIF,Cyt-C and Apaf-1 levels,PCI group,Cyt-C AIF preoperative and Apaf-1 level CAG group was obviously higher than that of coronary artery disease extent hints are positive correlated with cell apoptosis.After coronary interention may appear transient ischemia,increase cardiac injury Cyt-C and AIF,Apaf-1 as apoptosis of monitoring index PCI and help patients with coronary artery disease is not typical of myocardial injury after diagnosis.AIF may be reflected the sensitive cell damage indexes.
