3种促卵泡生长激素在长方案体外受精-胚胎移植中的效果 被引量:1

Effects of three long protocols of follicular stimulating hormone on in vitro fertilization-embryo transfer

摘要 目的:探讨在长方案降调节体外受精-胚胎移植中,不同种类FSH在临床使用中效果差别。方法:回顾分析2009年8月2010年2月在华中科技大学同济医学院附属同济医院生殖中心首次接受体外受精-胚胎移植(IVF/ICSI-ET)助孕术的病例共875个周期的临床指标。根据所用FSH不同分为3组:A组(果纳芬组),B组(普利康组),C组(福特蒙组)。比较各组获卵数、MⅡ卵数、2倍体受精卵数、获得胚胎数、FSH时间、FSH量、妊娠率是否有差异。结果:C组获卵数、MⅡ卵数、2倍体受精卵数、获得胚胎数明显少于A、B组,FSH量明显多于A、B组,有统计学差异。C组总妊娠率、临床妊娠率、持续妊娠率比A、B组高,无统计学差异。移植取消率C组最低,无统计学差异。OHSS发生率C组明显低于A、B组,有统计学差异。结论:rFSH较hp-uFSH获卵更多,但易导致OHSS发生率升高。各种FSH妊娠率无统计学差异。 Objective:To explore the differences of effects of different types of follicular stimulating hormone(FSH) in in vitro fertilization-embryo transfer(IVF-ET).Methods:The clinical indexes of 875 cycles of the cases who received IVF-ET for the first time in the hospital from August 2009 to February 2010 were analyzed retrospectively;they were divided into three groups according to FSH levels: group A(Gonal-F group),group B(Pouliquen group) and group C(Fostimon group);the numbers of retrieved oocytes,the numbers of MⅡ oocytes,the numbers of diploid oosperms,the numbers of embryos,times of using FSH,doses of FSH and pregnancy rates were compared among different groups.Results:The number of retrieved oocytes,the number of MⅡ oocytes,the number of diploid oosperms and the number of embryos in group C were significantly less than those in group A and group B,the dose of FSH in group C was significantly more than those in group A and group B,there was significant difference;the total pregnancy rate,clinical pregnancy rate and persistent pregnancy rate in group C were higher than those in group A and group B,there was no significant difference;the cancel rate of transplantation in group C was the lowest,but there was no significant difference among the three groups;the incidence of OHSS in group C was lower than those in group A and group B,there was significant difference.Conclusion:The number of retrieved oocytes in rFSH is more than that in hp-uFSH,but rFSH is easy to induce the increase of OHSS incidence;there is no significant difference in various pregnancy rates among the three groups.
