三维适形放疗联合奥沙利铂同步化疗治疗局部晚期直肠癌 被引量:6

摘要 目的观察三维适形放疗联合奥沙利铂为主化疗方案同步治疗局部晚期直肠癌的近期疗效及毒副反应。方法 52例局部晚期直肠癌患者接受放疗盆腔常规放射治疗40 Gy,再针对局部肿瘤三维适形放疗加量20-30 Gy。实验组(27例)同步行放化疗,静脉滴注奥沙利铂(L-OHP)130 mg/m2,第1天;氟尿嘧啶(5-FU)300 mg/m2,连用5天;亚叶酸钙(CF)200 mg/m2,连用5天;3周为1个周期。对照组(27例)单纯放疗。结果放疗结束后实验组和对照组总有效率(CR+PR)分别为74.1%和60.0%,两组比较差异无统计学意义(P=0.28);1、2、3年生存率分别为85.0%、67.4%、34.5%和75.8%、40.7%、17.0%,实验组总生存率明显高于对照组(P=0.046);远处转移率分别为25.9%和48.0%,两组比较差异无统计学意义(2χ=2.72,P=0.09)。实验组主要毒副反应是骨髓抑制、消化道反应和神经毒性,比对照组明显(P〈0.05);放疗引起的主要毒副反应是放射性直肠炎、膀胱炎等,两组比较差异不大(P〉0.05)。结论三维适形放疗联合奥沙利铂为主化疗方案同步治疗局部晚期直肠癌可提高生存率,毒副反应可以耐受,是一种有效和比较安全的综合治疗方法。
