乌司他丁对食管癌根治术单肺通气后炎性反应调控的影响 被引量:4

摘要 目的:探讨预注乌司他丁进行预防性干预对食管癌根治术单肺通气后肺组织氧化应激损伤的影响。方法:将全麻单肺通气下食管癌根治术患者32例,随机均分为预注乌司他丁组(u组)和对照组(D组)。u组麻醉诱导后静脉泵注乌司他丁10000U/kg,30min内注完;D组给予同量生理盐水。两组分别在麻醉诱导后(T0)、单肺通气1h(T1)、术毕(T2),术后24h(T3)4个时点采静脉血行血浆白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α),C-反应蛋白(CRP)检测。结果:两组各指标在整个手术过程呈现进行性上升的趋势,但u组各指标上升均明显低于D组。结论:预注乌司他丁可抑制单肺通气肺萎陷时TNF-α、IL-6、IL-8和CRP的过度释放,调控其炎性反应,从而减轻开胸侧萎陷肺的损伤,达到对肺组织的保护作用。
