摘要
目的 研制小儿罗叶泵用于小儿心室辅助,评估它在动物实验中的初步应用.方法 14头山羊,体质量(14.8±1.4)kg,随机分为小儿罗叶泵组(L-Y组,n=7)和柏林心脏Excor泵组(BH组,n=7).分别在降主动脉近段和左房插F14动脉插管和F22静脉引流管,接心室辅助泵的出入口,使用Medos驱动装置控制泵的参数.分别于心室辅助前、心室辅助2、4、6 h,记录控制参数、平均动脉压、心率的变化,检测血小板颗粒膜蛋白、游离血红蛋白以及肝肾功能指标.结果 L-Y组的正压参数(103±4)mm Hg,低于BH组的(165±22)mm Hg(P<0.05),负压参数差异无统计学意义(P>0.05).L-Y组流量为(1.20±0.20)L/min,显著高于BH组(0.94±0.22)L/min(P<0.05).心室辅助过程中两组动物平均动脉压和心率差异无统计学意义(P>0.05).两组血小板颗粒膜蛋白、游离血红蛋白含量没有明显变化.尿素氮(BUN)、肌酐(Cr)、总胆红素(TIB)、门冬氨酸转移酶(AST)、乳酸(Lac)含量在两组之间的差异无统计学意义(P>0.05).结论 小儿罗叶泵同柏林心脏Excor泵一样对血压和心率的影响较小,对血液及肝肾功能的破坏较少,在控制参数方面优于柏林心脏Excor泵.
Objective To evaluate the pediatric Luo-Ye pneumonic ventricular assist device (Luo-Ye pump) as a left ventricular assist device in the goat.Methods Fourteen goats weighing ( 14.8 ±1.4) kg were randomly individed into Luo-Ye pump group ( L-Y group,n = 7 ) and Berlin heart Excor group (BH group,n =7).Proximal descending aorta and left atrium were cannulated with Fl4 aortic cannula and F22 venous cannula respectively,and then cauunlas were connected with the pumps which were controlled by Medos drive unit.The mean blood pressure,heart rate and driving data were recorded during experiment.The serum platelet granulate membrane protein,free hemoglobin,hepatic and renal functions were assayed.Results The positive pressure of drive unit in L-Y group was less than that in BH group ( 103 ±4) mm Hg vs ( 165 ± 22) mm Hg,P〈O.05.The flow rate in L-Y group was higher than in BH group ( 1.20 ±0.20) L/min vs (0.94 ±0.22) L/min,P 〈0.05.The mean blood pressure and heart rate had no signiftcant changes during the experiment in two groups.There was no significant difference between two groups in the serum levels of platelet granulate membrane protein and free hemoglobin.No significant difference was found in the hepatic and renal functions between two groups.Conclusion The Luo-Ye pump demonstrated good initial hemodynamic performance and less injury to the blood,hepatic and renal function as Berlin heart Excor did.Furthermore,the driving date of Luo-Ye pump was better than that of Berlin heart Excor.
