下腔静脉滤器在下肢深静脉血栓形成治疗中的临床应用分析 被引量:13

摘要 目的探讨置入下腔静脉滤器在下肢深静脉血栓形成治疗中的临床应用价值。方法回顾性分析自2004年1月至2008年5月我科共收治的244例下肢深静脉血栓形成患者的临床及随访资料。结果下腔静脉滤器未置入组与置入组肺动脉栓塞发生率的差异有统计学意义(分别为8.19%和0%,P<0.05)。未置入组中有23例(23/196)患者因深静脉血栓形成复发再次住院治疗,下腔静脉滤器置入组中有6例(6/48)患者复发,两组之差异无统计学意义(P>0.05)。下腔静脉滤器置入组的肺栓塞发生率显著降低,但其中1例滤器倾斜,2例滤器移位,2例下腔静脉血栓形成,4例入路侧深静脉血栓形成和3例穿刺点血肿形成。结论下腔静脉滤器近期预防下肢深静脉血栓引起的肺栓塞简便有效,但其近、远期并发症应引起重视;其在下肢深静脉血栓形成治疗中的临床应用价值有待进一步评估和探讨。 Objective To study the clinical value of inferior vena cava filter in the treatment of lower extremity deep venous thrombosis.Methods Clinical and follow-up data from patients with lower extremity deep venous thrombosis(n=244)who were hospitalized in our unit between January 2004 and May 2008 were retrospectively analyzed.Results The incidence rate of pulmonary embolism was significantly different between vena cava filter placement group and control group(0.0%vs.8.19%,P0.05).Twenty-three(23/196)patients in control group were re-hospitalized due to the recurrence of deep venous thrombosis,whereas six(6/48)patients in inferior vena cava filter placement group had recurrence,showing a statistically insignificant difference(P0.05).The inferior vena cava filter placement group exhibited a significantly lower rate of pulmonary embolism,in addition to filter deviation(n=1),filter shift(n=2),inferior vena cava thrombosis(n=2),deep venous thrombosis of approach pathway(n=4),and hematoma of puncture site(n=3).Conclusion The short-term use of inferior vena cava filter is simple and effective for the prevention of pulmonary embolism caused by lower limb deep venous thrombosis,whereas the short-and long-term complications should be recognized.The clinical value for the treatment of lower limb deep venous thrombosis remains yet to be evaluated and studied.
