摘要
目的:分析研究中国妇女乳腺癌PTEN蛋白表达与组织学分级及淋巴结转移的相关性。方法:在中国期刊全文数据库、万方数据库、维普数据库、PubMed检索2003-2009年关于"中国妇女乳腺癌PTEN蛋白表达与组织学分级及(或)淋巴结转移相关性"的文献资料,结合作者近期相关的研究结果,然后对这些文献进行入选标准及质量的评价,并对符合标准的文献进行Meta分析。结果:15篇文献(1 212例患者)符合入选标准,组织学分级为Ⅰ、Ⅱ级组668例中PTEN蛋白阳性表达351例,组织学分级为Ⅲ级组184例中PTEN蛋白阳性表达57例,优势比为2.67,95%CI为1.48~4.80,P=0.001。有淋巴结转移组606例中PTEN蛋白阳性表达244例,无淋巴结转移组590例中PTEN蛋白阳性表达419例,优势比为0.30,95%CI为0.18~0.50,P<0.000 01。结论:在中国妇女乳腺癌中,组织学分级低级别组(Ⅰ、Ⅱ级)及无淋巴结转移组的PTEN蛋白表达较高,组织学分级高级别组(Ⅲ级)及有淋巴结转移组的PTEN蛋白表达较低。PTEN蛋白检测对于乳腺癌的诊断及规范化、个体化治疗具有重要的临床意义。
OBJECTIVE:To investigate the relationship between the expression of PTEN protein and histologic grade,lymph node metastasis in Chinese breast cancer patients.METHODS:This research retrieved the literature concerning the investigation of the expression of PTEN protein associated with histologic grade and/or lymph node metastasis in Chinese breast cancer between 2003 and 2009 from the Chinese periodical full-text data base,Wanfang database,Weipu database,PubMed,and from author's recently related study.The qualities of these data were evaluated,and the data meeting the selected standard were collected.A meta-analysis was performed.RESULTS:Fifteen reports,including 1 212 patients,were selected based on the standard.Of 668 cases of histologic grade Ⅰ,Ⅱ,351 cases were positive to PTEN;Of 184 cases of histologic grade Ⅲ,57 cases were positive to PTEN(OR=2.67;95%CI:1.48-4.80;P=0.001).Of 606 cases of lymph node metastasis,244 cases were positive to PTEN;Of 590 cases of no lymph node metastasis,419 cases were positive to PTEN(OR=0.30;95%CI:0.18-0.50;P〈0.000 01).CONCLUSIONS:The higher expression of PTEN protein is seen in cancer tissues with low histologic grade(Ⅰ,Ⅱ) and no lymph node metastasis among Chinese breast cancer,and vice versa.The detection of PTEN expression in breast cancer may possess important clinical significance to diagnosis and normalized,personalized treatment.
