摘要
目的严重创伤可导致机体炎症及免疫反应失调,并由此引起严重感染、多脏器功能衰竭甚至死亡。本研究旨在发现继发于严重胸部外伤的CD4+T淋巴细胞向Th1、Th2细胞异常分化等免疫失衡现象。方法采用细胞内因子染色及流式细胞术等方法对2009至2010年收治的10例严重胸部外伤患者(APACHⅡ20.5±6.6)伤后7天的外周血CD4+T细胞向Th1/Th2细胞分化,并分别以10例健康人及10例胸部轻伤患者(APACHⅡ4.5±1.5)作为对照。结果相较于健康人,重伤组CD4+T细胞向Th1、Th2分化均明显增高(Th1:13.8±2.6%,P≤0.01、Th2:4.9±2.2%,P≤0.05),但Th1/Th2分化的比例明显下降(Th1/Th2:2.82±0.6,P≤0.01),健康人与轻伤组患者无明显差异。结论严重胸部外伤破坏机体Th1/Th2反应的平衡,其可能成为机体细胞免疫功能紊乱、继发严重感染的机制之一。
Objective Severe trauma has been associated with inflammation and immunodisorders,leading to multiorgan dysfunction and death.In this research,we plan to observe CD4+T lymphocyte differentiation and the balance of Th1 and Th2 responses correlated with thoracic injury.Methods Peripheral blood was withdrawn from 10 severe thoracic injury patients with APACHⅡscores 20.5±6.6 on day 7 after trauma for analyzing the differentiation of CD4+ T cells,which were compared with 10 mild thoracic injury patients(APACHⅡ:4.5±1.5) and 10 healthy individuals.Results Compared with healthy individuals,the ratio of Th1:Th2 responses in patients with severe thoracic injury was significantly reduced(Th1/Th2:2.82±0.6,P≤0.01),and there was no significant difference between mild thoracic injury patients and healthy individuals.Conclusion These data demonstrated that severe thoracic injury destroyed the balance of Th1 and Th2 responses,which may result in immunosuppression,multiorgan dysfunction and death.
