摘要
目的探讨早期骨移植对掌指(趾)骨开放性骨折骨缺损治疗的临床疗效。方法 42例开放性掌指(趾)骨折骨缺损患者,26例早期分别采用自体骨移植治疗,占61.9%;16例行自体骨移植或异体骨移植加邻位皮瓣移植修复治疗,占38.1%。其中26例行一期清创并植骨,16例在一期术后8~12天内闭合伤口并同时植骨。结果 42例病例经4~10个月随访(平均7个月),41例伤口一期愈合(占96.72%),仅1例出现创面感染行对症处理后愈合(占2.38%)。所有掌指(趾)骨折均愈合,骨缺损修复时间为13~27周,平均20.5周。结论早期行骨移植治疗掌指(趾)骨开放性骨折骨缺损取得了理想的临床疗效。
Objective Study of early bone grafting in treatment of open fracture of the metacarpophalangeal (toe) with bone defects. Methods 42 patients with open fracture of metaearpophalangeal (toe) with bone defects, 26 cases (61. 9% ) of them were treated with early autologous bone grafting, 16 cases(38.1% ) of them were treated with vicinal flap repairing plus autologous or allograft bone grafting. The former group received routine debridement and bone grafting, the latter received wound closuer and bone grafting 8 -12 days later. Results All the cases were followed up for 4 - 10 months ( mean 7 mouths) postoperatively, 41 patients (96.72%) were successful at primary repair, only 1 symptomatic patient (2.38%) had wound infection, and then healed after treatment. All the metacarpophalangeal (toe) fractures were healed and the repair time of bond defect was 13 - 27 weeks, with an average 20.5 weeks. Conclusion Early implementation of bone grafting in open fracture of metacarpal (toe) with bone defect will achieve good clinical effects.
