泪道栓治疗准分子激光角膜屈光术后干眼疗效评价 被引量:7

摘要 目的研究可降解型VisiPlug泪道栓治疗准分子激光术后干眼的临床疗效。方法选取来我科就诊的23例(46只眼)拟行准分子激光手术且术前伴有轻到中度干眼患者,术后应用VisiPlug泪道栓进行泪道栓塞。分别于准分子激光术前、泪道栓塞术前、栓塞术后2周及3个月,针对干眼症状及泪道栓使用舒适度进行问卷调查,并行角膜荧光素染色、基础泪液分泌试验(SchirmerⅠtest,SⅠt)及泪膜破裂时间(break-uptime,BUT)检查,对疗效进行评价。结果准分子激光术后,患者干眼症状明显加重,SⅠt和BUT明显下降;泪道栓塞术后患者干眼症状明显减轻,SⅠt和BUT明显增加和延长,人工泪液使用频次下降;术后3个月所有患者角膜荧光素染色阴性。随访期间除1只眼栓塞术后1d泪道栓脱落外,无其他并发症发生。结论应用VisiPlug泪道栓可安全、有效地治疗准分子激光术后干眼,增加眼表泪液量,改善泪膜稳定性,缓解干眼症状,替代人工泪液的治疗。 Objective To evaluate the clinical efficacy of degradable VisiPlug lacrimal plug on dry eyes after refractive surgery. Methods Forty-six eyes of 23 patients with mild to moderate dry eyes were managed with VisiPlug lacrimal plug after refractive surgery. Questionnaires based on dry eye symptom and comfort degree to lacrimal plug, Schirmer Ⅰ test (SⅠt), fluorescent staining and tear break-up time (BUT) were performed before refractive surgery, before plug insertion, 2 weeks and 3 months after insertion respectively. Results The eye symptom got worse in all patients with SⅠt and BUT decreasing obviously after the refractive surgery but relieved significantly after the application of VisiPlug with Sit and BUT increasing and the frequency of artificial tears application diminishing. Three months after insertion, no fluorescent stainings of corneal epithelium were found in all eyes. Except for a spontaneous plug loss in 1 eye in the next day after the insertion, no other complications were observed. Conclusions VisiPlug lacrimal plug could be effective and safe in the management of dry eyes after refractive surgery by increasing tear production and improving of tear-film stability.
