摘要
目的比较高速涡轮钻微创法与传统凿骨劈冠法在低位埋伏下颌阻生智齿拔除中的临床效果。方法选择口腔门诊142颗下颌埋伏阻生智齿,随机分为微创组与对照组。微创组用高速涡轮牙钻去除骨阻力并分割牙齿,拔除低位埋伏阻生智齿71例;对照组用传统的凿骨劈冠法拔除低位埋伏阻生智齿71例。两组患者术后牙槽窝内均未放入任何药物,严密缝合拔牙创面。术后常规给予消炎药物,拔牙术后1、3、7 d各复诊1次,并记录相应的主诉、症状和体征。结果微创组患者术后疼痛、肿胀、张口受限反应较轻,舌侧骨板折裂和干槽症的发生率明显低于传统的凿骨劈冠法。结论使用高速涡轮钻微创拔除下颌低位阻生智齿,手术难度降低,创伤小,操作简单,可明显降低术后并发症的发生,是一种优于传统劈冠凿骨法的较安全、可靠的拔牙方法,值得基层医院开展。
Aim To contrast the clinical effect of atraumatic extraction and traditional extraction for mandibular impacted teeth.Methods 142 impacted mandibular wisdom teeth were randomly divided into two groups:atraumatic group(turbine drill exodontia)and control group(traditional hammer and chisel exodontia).Then clinical complications were observed at postoperation 1st,3rd,7th day.Results Complications of postoperative pain,swelling and trismus had significant differences in two groups,especially break of lingual alveolar bone and dry socket.Conclusion Turbine drill applied in extraction of impacted teeth can efficiently cut complications of operation and postoperation.Therefore,atraumatic extraction is recommendable.
