摘要
目的通过观察胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)及其受体(insulin-likegrowth factor receptors-1,IGF-1R)、胰岛素(insulin,INS)、雌二醇(estradiol,E_2)及其受体(estrogen receptor,ER)、孕激素受体(erogesterone receptor,PR)在子宫内膜息肉(endometrial polyps,EP)、息肉周围内膜和正常子宫内膜中的表达情况,从内分泌角度探讨EP的发病机制。方法选取2006年12月—2007年10月就诊于河北医科大学第二医院妇产科的生育年龄EP患者52例为研究组,同期因其他因素就诊的不孕妇女43例为对照组。采用酶联免疫吸附法和化学发光法检测患者的EP组织、息肉周围内膜和正常子宫内膜组织中IGF-1和E_2的水平;采用免疫组织化学SP法检测3种组织中的IGF-1R、INS、ER、PR的表达。结果①在增殖期亚组,EP的腺体和(或)间质中IGF-1、IGF-1R、INS的表达明显低于对照组(P<0.05),在周围内膜间质中INS的表达明显低于对照组(P<0.05);在分泌期亚组,EP与周围内膜的腺体中IGF-1、INS表达均明显低于对照组(P<0.05)。②在增殖期亚组中,ER、PR在3组腺体和间质中的表达差异无统计学意义(P>0.05);在分泌期亚组,EP间质中PR的表达明显低于对照组(P<0.05)。结论①子宫内膜息肉及其周围内膜组织中IGF-1、IGF-1R和INS表达的减少可能与EP的发病有关。②息肉间质中PR表达过低可能削弱了孕酮对内膜持续增生的负调控作用,从而促进了息肉的形成。
Objective To detect the expressions of insulin - like growth factor - 1 ( IGF - 1 ), insulin - like growth factor receptors - 1 ( IGF - 1 R), insulin (INS), estradiol ( E2 ), estrogen receptor (ER) and progesterone receptor (PR) in endometrial polyps (EP), adjacent endometrium and normal endometrium; to explore the mechanisms of EP pathogenesis. Methods All patients were chosen from the second hospital of Hebei medical university during December 2006 to October 2007. Fifty - two reproductive - age women benign endometrial polyp were recruited as study group and 43 infertile women without pathological changes of endometrium were recruited as control group. The concentrations of IGF- 1 and E2 were detected by enzyme linked immunosorbent assay and chemiluminescent immunoassay. The expressions of IGF - 1, IGF - 1R, INS, ER and PR were detected by immunohistochemistry. Results ①In the proliferative phase group, the expressions of IGF - 1, IGF - 1 R, and INS in the gland and/or stroma of EP were significantly lower than those in the control group (P 〈 0.05 ), and the expression of INS in the stroma of adjacent endometrium was significantly lower than that in the control group (P 〈 0.05 ). In the secretory phase group, the expressions of IGF - 1 and INS in the glands of EP and its adjacent endometrium were significantly lower than those in the control group (P 〈 0.05). ②In the proliferative phase group, the expressions of ER and PR in gland and stroma of the three parts showed no significant difference (P 〉 0.05 ) ; In the secretory phase group, the expression of PR in the stroma of EP was significantly lower than that in the control group (P 〈 0. 05 ). Conclusion ①The decreased expressions of IGF - 1, IGF - 1R and INS in EP and its adjacent endometrium are related with EP. ②The expression of PR in EP is suggested to be responsible for persistent growth of endometrium and pathogenesis of EP.
