摘要
目的:探讨脑血流动力学在新生儿缺氧缺血性脑病(HIE)中的动态变化及其在HIE诊断治疗中的临床意义。方法:采用彩色多普勒超声诊断仪对观察组HIE患儿54例和正常对照组50例于生后不同时段进行脑血流动力学指标的检测。结果:与对照组相比HIE组生后24、48、72h内各组动脉流速均降低,而阻力指数较对照组增高(P<0.05)。对比轻、中、重3组大脑动脉24、48、72、120h的血流动力学指标,发现阻力指数与病情呈正相关。结论:采用彩色多普勒超声检测HIE患儿大脑动脉的血流动力学参数能及时反映窒息新生儿脑血流的变化,有利于早期诊断HIE,及时制定干预措施,减少后遗症的发生。
Objective:To study the change of cerebrovascular hemodynamics and its clinical significance in diagnosis and treatment for newborn infants with hypoxic-ischemic encephalopathy(HIE).Methods:The hemodynamic indices of 54 cases of HIE and 50 controls within postnatal 24,48,72 and 120 h were measured by color Doppler ultrasonography.Results:Compared with those in control group,in HIE group the flow rate of cerebral arteries descended and the resistant index increased significantly within postnatal 24,48 and 72h(P0.05).As for hemodynamic indices of cerebral arteries in light,medium and severe HIE infants,there were positive correlation between resistant index and patient's condition.Conclusion:The observation of hemodynamics of asphyctic neonates will be in favor of the diagnosis and treatment for cerebral injury of hypoxic-ischemic encephalopathy neonates.
